Alice Ann leads Operations, Sales, Information Technology and Clinical Operations for Kindred at Home's Hospice division. She has been with the company for over ten years, co-founding CURO Health Services with Larry Graham in 2009. She is responsible for ensuring KPI's drive operational strategy, aligning high capacity teams with strategic goals, and implementing the hospice division's platform acquisition integration and de novo strategies. Prior to CURO and Kindred at Home, Alice Ann served in a dual executive role as CIO and Senior Vice President of Clinical Operations at Amedisys. She received her Bachelors of Science in Nursing from Johns Hopkins University and is a 2019 MBA Graduate of The Massachusetts Institute of Technology (MIT).

- Chief Information Officer
Craig joined the company as our Chief Information Officer in October 2018. With over 24 years of information technology leadership in healthcare, Craig also has deep and diverse expertise on payer, provider, software and services transformation. Craig comes to Kindred At Home from Humana where he most recently held the position of Vice President of Technology, Mergers & Acquisitions. Prior to this, he held roles as CTO for Conifer Health Solution's Value Based Care division, CIO of Concentra, and numerous technology leadership roles at Blue Shield of California. Craig holds a bachelor's degree with an emphasis in Corporate Finance from the University of Southern California, and an MBA with an emphasis in Health Services Administration from Golden Gate University.
Since joining Gentiva/Kindred at Home in 2003, David has served in various senior leadership roles including Senior Vice President of Operations and President for the Home Health Division. Starting in May 2014, David assumed the role of Gentiva/Kindred at Home's President after serving as the COO and Executive Vice President. Prior to joining Gentiva/Kindred at Home, he was the COO of Millennium Health Care Resources. David holds a nursing degree from Presbyterian College and a bachelor's degree in Health Care Administration from the University of Phoenix.
Dean Johnson - Chief Commercial Officer
Dean has 24 years of domestic and international sales and marketing leadership experience in the post-acute care service environment. He has been with Kindred at Home since 2010 in various key leadership roles, including Division Vice President of Sales and Marketing, Home Health; Senior Vice President of Sales and Marketing for Home Health and Hospice; Senior Vice President of Enterprise Sales for Kindred; and Chief Commercial Officer for Kindred at Home. Dean graduated from the University of Colorado, Boulder with a bachelor's degree in Economics.
- Chief Clinical Operations Officer
As Chief Clinical Operations Officer, Derek has responsibility for all combined Kindred at Home clinical products, development, analytics, quality, delivery and service. His duties also include the coordination and leadership of the developing clinical collaboration with Humana. Derek previously held multiple clinical and operational positions over his 14 year tenure with Kindred at Home and its subsidiaries, ranging from a field clinician to Regional Vice President of Clinical and, most recently, Vice President of Operations. Prior to joining Kindred at Home, he spent five years in an outpatient private practice. Derek completed his undergraduate degree at Lock Haven University, his Masters of Physical Therapy from Emory University School of Medicine, and his Clinical Doctorate from the University of Montana.

Heather O'Sullivan's 20 plus years of healthcare experience spans both clinical experience as a geriatric nurse practitioner and clinical operations in senior leadership roles at a number of the nation's leading post-acute care organizations, health plans and home healthcare organizations. Heather provides strategic clinical leadership and direction to the organization - developing, implementing, and advancing Kindred at Home's patient-centered care delivery models. Heather earned her BSN from the Medical University of South Carolina and her Master of Science in Nursing from Simmons College. She serves as a Corporate Advisor to the American Organization of Nursing Leadership.

Kristen has nearly 20 years of experience in healthcare. She joined Gentiva/Kindred at Home in 1997 as a Senior Financial Analyst and served in multiple financial roles with increasing responsibility, including Director of Finance for Gentiva/Kindred at Home's consulting division, Assistant Vice President of Finance, Vice President of Corporate Finance, and Vice President of Strategic Initiatives. Kristen played a lead role in the Kindred at Home/Gentiva integration and her leadership was integral to its success. She also has done a tremendous job leading the business intelligence (BI) initiative and overseeing new acquisitions for the division. Prior to the transaction that established Kindred at Home as a standalone company, Kristen served as Senior Vice President and Chief Administrative Officer, with one of her key areas of focus being the enhancement of the employee experience for all Kindred at Home team members.

Larry is the co-founder of CURO Health Services, and he most recently served as the company's CEO. Since its inception in 2010, CURO has grown to be one of the largest private hospice care providers in the United States. With a strong company culture committed to compliance and clinical excellence, Larry assembled a management team that successfully completed six acquisitions and more than 100 start-up agencies. Prior to founding CURO, Larry served as the President and Chief Operating Officer of Amedisys. During his tenure, Amedisys experienced increased revenue from $25 million to annualized revenue in excess of$ 1 billion in 2009.
Larry has played a key role in the development of charitable organizations including the Grace Hospice Foundation at CURO as well as the Christen Foundation at Amedisys. These organizations were created as a means to give back in the communities they serve. Prior to entering the home health and hospice industry, Larry served as Director of Financial Services with General Health System and as a Senior Accountant at Arthur Andersen.

As Chief Human Resource Officer, Mark's focus is on continuing to integrate the company's business functions, creating a high performing leadership team and implementing a best in class employee experience for the 50,000 people at Kindred at Home. Mark has more than 30 years of experience in strategic human resources leadership. Prior to joining Kindred at Home, Mark was the Executive Vice President and Chief Human Resources Officer for USAA. He has also held leadership roles at Diageo PLC, CDI, General Electric and RCA Corporation.
Ron Lazas - Chief Legal Officer
Ron began his career at Kindred Healthcare in 1994 after spending four years in the commercial transactions and finance group of Frost Brown Todd, LLC. Prior to his appointment as General Counsel, Ron was Kindred's Senior Vice President and Deputy General Counsel responsible for coordinating the operational legal services for each of Kindred's operating divisions. Ron is a graduate of the University of Wisconsin Law School.
With more than 30 years of experience in the healthcare industry and nearly 20 years of service with Kindred, Selece has a strong background as an RN, as well as extensive legal and compliance expertise. She most recently was Kindred at Home's Senior Vice President and Chief Clinical Officer. She previously served as Kindred at Home's Divisional Vice President, Regulatory Affairs and Clinical Risk, working to support the company through the interpretation and education of regulatory changes, with the goal of minimizing financial and patient care risks. Selece holds a bachelor's degree in Nursing from Howard University, and she earned her Juris Doctorate from North Carolina Central University.
Tom joined the company as Chief Financial Officer in October 2018, overseeing all finance-related activities for the enterprise. In this role, Tom brings 30 years of banking, accounting, and corporate finance experience, including over 13 years in healthcare experience. Before joining Kindred at Home, Tom served as CFO of Schumacher Clinical Partners, a private equity-backed specialty physician management company with over $1 billion in revenue. Previously, he was Senior Vice President of Finance and Treasurer for Amedisys, Inc., a leading provider of home health and hospice services. Tom earned a bachelor's degree in Business Administration and an MBA from the University of Michigan.
