The plan outlines the type, frequency and duration of service required to meet the needs of the client and is monitored on an ongoing basis to ensure service is appropriate and coordinated... Avec Care has been providing services tailored to meet the needs of clients requiring care in their own home and assisting them to continue living independently since 1986... Avec Care offers a wide range of services 24 hours a day, 7 days a week. Highly experienced Program Managers coordinate, plan and arrange the required services after a personal consultation. This provides a single point of contact, if additional assistance is required or if there are any concerns.
