The business began when two dentists in Atlanta, Georgia could not find the professional forms they needed for their growing office. Since then, Informs has grown to providing over 100 customized patient information forms in a wide range of fields to our customers. There are standard patient information/welcome forms to choose from or Informs can customize anything to fit your needs.
