One call to 1-877-HI-NYHCA, our Toll Free hotline provides hospital discharge planners, health plan case managers and families with access to an NYHCA nursing professional. NYHCA staff can help callers find the appropriate SNF within our network that meets the need of individuals who require sub-acute, rehabilitation and/or long term care placement... NYHCA formed a Group Purchasing Organization to assist member facilities in achieving a stronger bottom line through contract purchasing. Through a comprehensive contract portfolio, member facilities participate in a vast array of contracts carrying a multitude of products and services. NYHCA provides education and training to member facilities related to managed care and skilled nursing facilities.
