OTSI was established on 1 January 2004 as a separate division within the Independent Transport Safety and Reliability Regulator (ITSRR) to investigate and report on rail, bus and ferry safety incidents in NSW... OTSI is led by the Chief Investigator who is appointed by the Governor of NSW on the recommendation of the Minister for Transport. Jim Modrouvanos is currently acting Chief Investigator while recruitment is undertaken to permanently fill the role... OTSI conducts investigations, undertakes data analysis and research, and engages with stakeholders to share insights on risks to support safety improvements.
