1. To be eligible to participate in the Points of Focus program ("POF"), employees must (a) meet POF guidelines for job designation and route assignment; (b) complete 91 days of continuous employment with US Foods, Inc. ("Company"); (c) activate their online POF account at www.usfoods.com/pointsoffocus ("Account"); and (d) acknowledge and accept these Terms & Conditions, including Sections 3 and 4 below. In the event an employee is new to the Company or POF, the employee must also sign the Company's Non-Solicitation and Non-Disclosure Agreement ("NSNDA"). Employees are not required to sign a new NSNDA if the employee has signed one previously during the course of employment with the Company. Upon acknowledgement by the employee, these Terms & Conditions shall renew each year and continue in effect unless and until they are emended, modified, or terminated by the Company as set forth in Section 12 below... 4.
