AgeWell Solutions, a solo practice, is professionally led by Tracey Olson, a licensed clinical social worker Geriatric Care Manager in the Chicago area. Works with older adults, and persons with mental illness or physical disabilities... It was a wonderful opportunity to collaborate with so many clients and families at AgeWell Solutions these past ten years... Beginning in January 2022, I retired from Care Management. I appreciated coming into peoples' lives at extremely challenging times. Working with all the families, clients and the care teams to make sure everyone knows the care options available and is receiving the care they need has been gratifying.
