NURSE CASE MANAGER

Nurse Case Managers are in charge of coordinating long-term care for their patients. Case Managers and in-home care agencies often work closely together given their shared goal of keeping patients healthy and out of the hospital setting. After completing a comprehensive assessment of the patient, the case manager will create a care plan, often alongside the in-home care agency who will be overseeing the long-term care of the client. Nurse Case Managers, also referred to as Geriatric Care Managers or Senior Case Managers, work with patients and their families as well as in-home care agencies to help the patient reach and maintain his or her highest level of functioning.
