Health Care Connection is a suite of internet software tools designed to help arrange and coordinate post-hospital care and manage transitions by simplifying the communication process between care facilities, health care services, diagnostic services, and equipment providers. It is a low-cost, fee-per-use system that uses a secure HIPAA compliant database to store, manage, disseminate, and report patient information. The system is a suite of programs that are customized to simplify each step in the discharge process... I called my friend, Lowell Dunn, a data base expert, to see if we could come up with a solution. After years of work and discussions with Case Managers and hospital CEO's, we developed an internet based method of automating the discharge process that is fast, inexpensive, easy to use and simple to install. In addition, we wanted to address the problem of discharging out of town, and coordinating with family members and doctors... Creating more time for Patient Care...
