The information accessible through MyVMC includes confidential client/patient information. Only the client of record, and persons specifically authorized by the client of record, may access that information. By accessing MyVMC, I acknowledge and agree that I am the client of record, or a person authorized by the client of record to access confidential client/patient information. I also acknowledge and agree that I may not disclose, transmit, publish, or copy the client/patient information on MyVMC, including digital images, without the express authorization of the client of record and the VMC.
