Case management is a vital service because it ensures that any person requiring a number of different health and social services will have these coordinated. This is especially important if someone is being returned to the community; perhaps after mental health issues or medical health difficulties have meant that they have been hospitalised for awhile. Failure to have all the right services in place could have grave consequences and could mean that the individual will need to return to a care setting.
