We care for the physical needs of each patient. Hospice of the Sacred Heart is committed to alleviating distressing symptoms and minimizing pain, while keeping the patient in control. Guided by our interdisciplinary plan of care, our staff is available 24 hours a day, seven days a week, usually in the patient's home-with family members and friends serving as the primary, hands-on caregivers. However, hospice services are also available to patients in nursing homes and assisted living facilities. Hospice nurses educate family members to care for the patient properly and safely. During the time the patient is under hospice care, our nurses keep the primary physician, our Medical Director and the family informed about the patient's condition...
