The Woodbridge Medical Centre Family Health Team (WMC FHT) is a collaborative primary health care team composed of a Dietitian, Family Physicians, Family Practice Nurses, Nurse Practitioners, Pharmacist, Psychotherapists, Social Worker and other Professionals. We work together as an interdisciplinary team, to provide patient centered primary health care services and programs... Our goal is to improve the current and changing healthcare needs of the community by providing the highest quality health care services with a collaborative and integrative approach... WMC FHT aims to provide efficient, high quality health care services in the community with a focus on improving patient access, health promotion and chronic disease management.
