Founded in 1997 as a free clinic called Fox Cities Community Health Center, we went on to become a federally qualified health center (FQHC) in 2004 and, in 2013, we changed our name to Partnership Community Health Center. PCHC now cares for 15,000 patients annually and employs nearly 140 staff from the local community. Our care model includes primary medical, dental, integrated behavioral health and comprehensive outreach and enrollment services... Founded in 1997, Partnership Community Health Center has become a leader in affordable dental, medical, behavioral health services in Central Wisconsin.
