Care Coordination, a new way to connect to these services... LIFEPlan CCO is a partnership of community-based agencies with a long history of serving people with I/DD, providing advocacy, recreational, educational and vocational services. We share a person-centered focus that improves services and supports through networking with highly trained human service, healthcare and other specialists.
