Alyssa Gallone has been with Patient Care Coordination for over 3 years to date starting out as the Nurse Assessor and growing into her new position as the Director of Clinical Care Management. Alyssa brings Patient Care Coordination an extensive background of over 10 years in Direct Patient Practice, Nursing, Gerontology, Clinical Compliance and Auditing ranging in environments of acute care, care management, long term care, nursing and rehabilitation, memory care, medication management and disease oversight. Alyssa is a Registered Nurse and possesses a dual Bachelor's along with a Bachelor's in Nursing from a highly accredited institution in Philadelphia. Alyssa brings Patient Care Coordination a broad scope of skills and knowledge that promote wellness, preventive efforts along with emphasis on the individual.

Jardena Zitserman

Jardena Zitserman joins the Patient Care Coordination team and brings over 10 years experience in the Managed Long Term Services and Supports setting. Jardena brings PCC an extensive background in the healthcare setting ranging from MLTSS, Acute and Chronic Care Management, Service Coordination, Outpatient Medical Facility Management, and Urgent Care and TeleHealth Operations. Jardena possesses over 6 years of operational experience through direct management and operational oversight within both the LTSS and urgent care setting. Jardena was an integral part of the development and lead of the COVID Testing Sites along with transition to telehealth operations throughout the onset of the Coronavirus Pandemic with a leading Urgent Care Center provider in Philadelphia. Jardena maintains a Bachelor of Arts in Psychology from Temple University in Philadelphia, PA.

Paul S. Rimar

Paul Rimar established PCC as a Service Coordination agency in the fall of 2014. Paul began as the Executive Director, Service Coordinator, Administrative Assistant and Marketer. Paul believed that if he recruited the right team and stayed true to his vision, PCC would grow and lead the way for others to follow as a servant of the community and its vulnerable members. Paul instilled these values within PCC to ensure that waiver-eligible participants were served with integrity, dignity and compassion. PCC began with zero participants in a small Philadelphia office and quickly outgrew the space by mid-2015. PCC then established the central Philadelphia business hub to be centrally located and accessible for the majority of our participants being served. Through his experience and passion, Paul took a leap of faith to begin PCC as its founder and current Executive Director. His background in social work and experience working in a managed care setting inspired him towards achieving the healthcare "triple aim" (Improving the patient experience of care including quality and satisfaction; Improving the health of populations; and Reducing the per capita cost of health care) in the populations we serve. Paul spreads the mission and values of the organization through its staff, community, and the participants we manage. Paul brings Patient Care Coordination over 13 years of experience from a variety of facets including, but not limited to long and short term care, acute care hospital, hospice and palliative care, managed care, and nephrology and transplantation. Paul achieved his Masters in Social Work from Temple University along with a Certificate in Gerontology from Temple University School of Medicine - Institute on Aging.
