TW FHT

The Toronto Western Family Health Team (TW FHT) was established in 2007 and is built on a long-standing family medicine department at the Toronto Western Hospital and University Health Network (UHN). We are partnered with the Department of Family and Community Medicine (DFCM), an academic teaching unit fully affiliated with the University of Toronto. The TW FHT is overseen by its Board of Directors, which is responsible for its overall governance... TW FHT is an interprofessional team-based primary health care organization. This means that we have different health care professionals with unique skills and expertise that work together to give you the best possible care... The TW FHT is affiliated with the UHN Department of Family and Community Medicine, which trains University of Toronto medical students and medical residents, in addition to other health professions students.
