TERMS OF SERVICE: On behalf of the State of Ohio and the Ohio Department of Medicaid (the "State"), CSIS provides this case management software solution and related features, functionality and technologies (the "System") to users authorized and identified solely by the State ("User(s)") to use the System for State-specified uses regarding the Ohio Home Care Waiver and Specialized Recovery Services programs (the "Program"). The System contains confidential State data, such as case records ("State Data"), and State Data is the sole property of the State. The System is the sole property of CSIS. Users are responsible for accessing and using the System and State Data solely for authorized Program purposes, in compliance with applicable laws. Users are responsible for having executed and complying with a State Code of Responsibility (ODM 07078) that is in full force and effect.
