Integration Steps addresses the challenges in bridging Primary Care and Behavioral Health. Through online resources, this website serves as a tool for practices to build new screening and referral pathways, and develop new means of interdisciplinary communication. Our goal is to train providers and staff in all of these new initiatives, keep team members informed of updates, and provide opportunities for ongoing learning...
