Aaron is a licensed attorney focusing on organizational compliance and regulatory affairs for the HFPN. He brings over six years of contracting experience, three of which are in Medicaid, Medicare, and commercial payor contracts. A graduate from Western Michigan University Cooley Law School, Aaron has held multiple leadership appointments from American Bar Association and the State Bar of Michigan. Aaron has a Bachelor of Arts from Grand Valley State University and a Master of Science from the Johns Hopkins University. Aaron serves on Henry Ford Health's Ethics Board, and is a past winner of the Davidson Fellowship in Healthcare Innovation.

With over 15 years of healthcare project management experience, Abbi brings a wealth of knowledge and expertise to the HFPN. This vast experience includes leading large initiatives with executive stakeholders across multidisciplinary teams in the areas of Information Technology, Analytics, Physician Organizations with Value-based Care and hospital operations. With her background in both Agile and Waterfall methodologies, Abbi supports HFPN growth and success by leading the team through Agile project management process to maximize outcomes and reduce waste. Abbie serves as the system administrator of the PRM online tool being utilized by outreach liaisons across Henry Ford Health. She brings an abundance of healthcare experience to the HFPN, including insurance enrollment and eligibility, medical billing and claims payment, medical office operations, and data analytics. Over her 20-year career in healthcare, she has worked for United Healthcare, Intermountain Healthcare, and Henry Ford Wyandotte Hospital.
With a wide array of experience within Henry Ford Health and beyond - including Salesforce CRM experience - Abby offers the Network a rich reservoir of skills and qualifications. Prior to joining the HFPN team, Abby supported several System Redesign and Transformation projects within the Pharmacy and Security teams to deliver systemwide cost savings. She brings with her a certificate in Business Process Model and Notation, along with Master Black Belt Certification in Lean Six Sigma from Lawrence Technological University. She received her Bachelor's degree in Health Sciences from Kalamazoo College.

The membership of the hospital's Board of Trustees had changed during and after the war, with the death of Edsel and the election of Henry II and Benson as members. Cyndy Lambert serves as the Senior PR Specialist for the HFPN. In her role, she is responsible for ongoing communications to all members of the HFPN and any material related to the Network. Cyndy has more than three decades of experience in writing and communication, with a background in newspapers (The Detroit News) and employee communications at Compuware Corporation. She also helped develop and manage a community-engagement health program for the Karmanos Cancer Institute, focusing on cancer prevention. Cyndy received her Bachelor's degree in journalism from Wayne State University.

Geoff Patterson brings over 30 years of healthcare experience, in clinical, technical, and administrative areas. He currently serves as the Chief Information Officer of the Henry Ford Health Care Delivery Network. In his current role, he is responsible for information technology strategy, optimization, and delivery across all of the health system inpatient and ambulatory locations.
Mr. Patterson joined Henry Ford Health in 2007 from Trinity Health where he served in various roles transforming clinical and operational systems. Previous positions include direct care delivery as a Respiratory Therapist, both in the inpatient and home care divisions for the Ascension St. John Health System.
Mr. Patterson received a BBA and MSBIT from Walsh College and has been a member of the College of Health Information Management Executives since 2008, serving on its membership and certification committees, and the Project Management Institute since 2006.

Hannah first joined the team as a Practice Transformation Consultant before moving to her role as Project Manager in 2023. In that role, Hannah focused on developing growth strategies for both Henry Ford Medical Group and private practice physicians. She now brings that experience to the expanded roster of providers in Mosaic. Jeanne Wymer has worked in healthcare since 1993 with a variety of roles in IT Leadership, Project Management, Application Management, and Programming. Jeanne's career focused on implementation of a community-wide electronic health record in 2005 with the creation of Jackson Community Medical Record. With a passion for improving the health of our local community, Community Health Technology Network expands on the groundwork established with Jackson Community Medical Record to grow into southeastern Michigan.
As the Director, Business Operations for Community Health Technology Network, Jeanne is responsible for the legal, financial, and outreach programs. Jeanne also prepared materials and presents to the CHTN Board of Trustees.
Jeanne developed, and remains responsible for, an IT Business Relationship Management team at Henry Ford Health Jackson to improve the IT experience for all our operational leaders. As the IT Site Director, Jeanne is responsible for high reliability IT within Henry Ford Health Jackson.

Jeff has applied his expertise in industrial engineering and analytics within several teams at Henry Ford Health; most recently, with the HFPN and its direct-to-employer contracts. Previously, Jeff worked within multiple departments at Henry Ford, including Transformation Consulting, General Internal Medicine, and Performance Analytics. While working at a healthcare consulting firm for seven years (2014-2021), Jeff supported projects funded by the Department of Defense. In this role, Jeff applied and grew his skills with client management, consulting, analytics and software development. He returned to Henry Ford Health in 2021 as Director of Direct-to-Employer (D2E) Relationships, overseeing the execution of key D2E contracts and helping design the organization's direct-to-employer strategy. Jeff graduated from Wayne State University with a Master's Degree in industrial engineering.

Kanchan brings a wealth of knowledge and experience to her role as a member of the HFPN Practice Transformation Consulting team. Her diverse background in patient care, healthcare administration and business make her a unique and welcome addition to the PTC team. As part of her background, Kanchan spent seven years working as a Physician/Administrator in India within hospital and clinic settings. She is committed to making a positive impact with the HFPN through working collaboratively with healthcare providers and staff to drive meaningful change.

Karyn Lemieux has worked in healthcare since 2012 in various roles supporting Epic EMR implementation including training, support, system build, and management. She has served in different positions within the Community Connect program at Henry Ford Health since 2014.
Karyn is responsible for the Henry Ford Health Community Connect program to build, deploy, and enhance the Epic EMR for affiliated private practice providers. Her team is also responsible for the perpetual support of Epic-using private providers in the Henry Ford Health North and South regions.

Napoleon has worked within the financial arena for years, lending his expertise to multiple nonprofit organizations, primarily in the healthcare industry. Prior to accepting his role as Financial Analyst with the HFPN, Napoleon served as a Financial Consultant for the Global Health Department at Henry Ford Health, where he managed two of the largest COVID-related clinical trials within the U.S. He received his Bachelor's degree in Business Administration from the University of Michigan-Flint and is now pursuing his MBA at Kettering University.

Rachel brings to the HFPN her extremely valuable perspective as a nurse who also has a keen understanding of analytics. A practicing nurse for five years, Rachel has worked in orthopedics, trauma and medical-surgical and now turns her attention to physician relationships and also to the finer points of Health Planet Link, a tool used to improve quality-based care.

Sharon brings an arsenal of tools and experience to the HFPN that complement the work already underway. Prior to joining the HFPN team in June 2021, Sharon was an Administrative Fellow at Henry Ford Health, serving in the Office of the CEO. Before that, she worked as a Clinical Research Analyst with the Ventricular Assist Device (VAD) team at Michigan Medicine. Additionally, she worked for the University of Michigan in multiple areas, including the Center for Value-Based Insurance Design and Center for Health Research & Transformation. Sharon holds a Masters' degree in Health Services Administration and a Bachelors' in Biopsychology, Cognition & Neuroscience - both from the University of Michigan.

Theresa has been the driving force in the development of educational forums for office managers within the HFPN and is instrumental in the planning of the annual HFPN Summit. She is currently pursuing a Masters' degree in public administration from the University of Michigan, Dearborn.

Therese has worked in health care since 1984 holding both clinical and administrative roles in Nursing and Clinical Informatics. She is currently serving as the Director of Community Health Technology Network, Central Market (CHTN) and Health Information Exchange for Henry Ford Health. In her various roles since 2007 at CHTN (formerly Jackson Community Medical Record), she has combined her clinical and IT background to oversee implementation, training, support, and the rollout of a community record to private physicians, employed physicians, and advanced practice providers emphasizing the need for a longitudinal record through health information exchange. CHTN has implemented Epic's Community Connect program as our electronic health record since 2018.
In her early nursing career, Therese held management roles in inpatient and outpatient settings. She was instrumental in the creation of several behavioral health programs during that time. Clinical informatics is a passion for Therese, and she has been involved in the use of data and technology to improve a patient's experience and create efficiencies in physician documentation for over two decades.
Therese is a member of the ANIA (American Nursing Informatics Association) and holds a Bachelor of Science in Nursing from Madonna University. Most recently, she was nominated to sit on Epic's 2022 Community Connect Steering Committee where she is excited about sharing ideas and advancing key concepts with other organizations.

Mr. Hazen currently serves as Transition Advisor for Jackson Health Network (JHN), a clinically integrated network in Jackson, Michigan. The Network is a collaboration of healthcare providers, physicians, and hospitals that recognize the need to transform both the delivery of healthcare as well as the financing of healthcare. We use the IHI Triple Aim concept, improving health outcomes, patient's experience of care and impacting the cost of care, as our driving principle.
Mr. Hazen also serves as an advisor to Community Health Technology Network (CHTN), a community-wide electronic health record system affiliated with Henry Ford Health.
Mr. Hazen has worked in health care management since 1990. His principal recent focus has been providing short-term project assistance in executive management, financial, and operational areas of leading health care entities. His work spans both for-profit and not-for-profit environments and his experience includes work at both start-up and turn around businesses. Since 2008, Wynn has served as a member of executive health plan management teams.
Earlier in his career, Mr. Hazen held long-term positions in the finance and operations departments at General Electric and Healthsource. He has also been a member of the adjunct SPEA faculty at Indiana University-Purdue University at Indianapolis teaching coursework in Health Care Finance and Budgeting.
Wynn received a BS in Business Administration from Valparaiso University and his MBA from St. Francis College.

Ziad serves as the principal data scientist for the Network, as well as Physician Relationship Management (PRM). In his role, he conducts analyses and develops statistical models to support strategic decision making. Prior to joining the HFPN, Ziad lead the operational launch of the Brigitte Harris Cancer Pavilion. He holds Bachelor's and Master's degrees in Engineering (with a focus on artificial intelligence and machine learning) - both from the University of Windsor (Ontario). He is currently pursuing a PhD in Engineering at the same institution.
