The Health Center, located in Plainfield, VT, is a primary care practice. As a Federally Qualified Health Center (FQHC), our primary service area includes the towns of Cabot, Calais, East Montpelier, Marshfield, Plainfield, and Woodbury, and we also serve patients from many other surrounding communities. We are a non-profit, community-based health center dedicated to providing quality health care for individuals and families regardless of ability to pay... The Health Center offers comprehensive high quality and patient oriented health services to meet community needs, regardless of ability to pay, and cognizant of the social determinants of health; provides health education to our patients and our community; and contributes to the progress of medicine through teaching, research, and writing.
