Our Partnership is made up of organisations working closely together to plan services and address the challenges facing health and care services across the area... Our Partnership is not a new organisation. It is a new way of working for the 2.4million people who live in Bradford District and Craven, Calderdale, Kirklees, Leeds and Wakefield... West Yorkshire Health and Care Partnership is an Integrated Care System. We work in partnership with NHS organisations, councils, Healthwatch, hospices, charities and the community voluntary and social enterprise sector to improve the health and wellbeing of local people living in our five local places; Bradford District and Craven, Calderdale, Kirklees, Leeds Wakefield District.
