Thus, we offer the highest level of coordinated care to everyone at every age. In fact, our team-based care ensures all providers are communicating, aligned and treating the whole person... Ability to pay should not factor into the quality of care you deserve. That is why we offer a program that utilizes a sliding scale based on your income and family size. Those eligible may receive reduced or discounted services. There is no charge to apply for the Sliding Fee Program. However, you must satisfy the eligibility requirements mandated by the federal government.
