Caitlin Parker

Caitlin Parker is an Accredited Practising Dietitian and a Credentialled Diabetes Educator at PDC Health Hub. She has completed a Bachelor of Science in Nutrition, Post Graduate Diploma in Dietetics and a Graduate Certificate in Diabetes at Curtin University. Caitlin has lived with type 1 diabetes since 1994. This, along with an interest in science led her to study to become a dietitian. Caitlin also works as a cardiac rehabilitation and aged care dietitian. Caitlin is passionate about helping people with diabetes. She knows there are many ways to improve health and blood glucose levels and will work with you to find the method that best suits you. Caitlin can assist you to make diet and lifestyle changes to improve health and blood glucose levels. Outside of work, Caitlin's hobbies include staying active with her children and 2 dogs, exercising at her CrossFit gym, enjoying time with family and friends, cooking (of course), and sewing clothes for her beautiful daughters!

Jake O'Brien - Chairman

Jake is also a member of several other local, state, and federal diabetes peer support and advocacy groups where he helps with peer support programs and guide policy changes. Outside of both work and medicine, Jake is an avid runner, coffee drinker and tech head. After having completed his first marathon at age 18, Jake has continued to prove diabetes is no barrier to achieving your life goals. He is incredibly passionate about exercise and the enormous benefits it has on the entire body. He's also probably drinking a cappuccino right now!

Jeffery Au

Jeff is a Registered Pharmacist and has been registered since 2006. In his career, Jeff has managed a busy retail pharmacy in Midland and a service based pharmacy in Joondanna. He has a strong passion for diabetes care and has provided education to young aspiring pharmacy interns on diabetes devices, rural pharmacists on diabetes services available in a pharmacy setting and pharmacists re-entering the workforce on the new and current trends of diabetes care in pharmacies.

Maxine Schlaeppi

Maxine is a Registered Endorsed Nurse Practitioner and Credentialled Diabetes Educator with over 29 years' experience in diabetes education and management. Maxine has worked in a vast range of environments -from paediatrics to adult diabetes. Maxine is a Certified Insulin Pump Trainer for all available insulin pumps in Australia and all Continuous Glucose Monitors (CGM). Since 2002, Maxine has worked with people living with Diabetes in Pregnancy (including Gestational diabetes and Type 1 Diabetes and Pregnancy). Maxine is a proud grandmother and her interests outside of her professional career include spending time with her grandchildren, travel, and continuing to learn more about technologies for diabetes.

Melissa Tong

Melissa is a pharmacist with over 10 years experience having completed her Bachelor of Pharmacy at Curtin University in 2008 and accredited as a certified geriatric pharmacist in 2018. Melissa has worked in the community at various pharmacies across WA with a focus in working in collaboration with patients and other health care professionals to optimise health outcomes.

Rachel Boen

Rachel is an Accredited Practicing Dietitian who enjoys cooking, baking and talking about food so much so that she turned it into a career! Rachel graduated with a Bachelor of Science (Nutrition) and continued to pursue a Masters of Dietetics at Curtin.

Simone Bishop

Simone Bishop is a registered Counselling Psychologist with experience in working with individuals, couples, children and families, and groups. Having graduated from Murdoch University in 2004 she later went on to obtain her master's degree in Psychology (Counselling) from Curtin University. In addition to private practice Simone has experience working in multidisciplinary treatment settings, including private psychiatric hospitals and within the child protection field. ​Simone holds experience working with a broad range of presenting issues, including symptoms of depression, anxiety and stress, relationships, parenting, attachment, grief and loss and working with individuals who have experienced trauma (whether from childhood, in the context of early attachments, or from later on in life). ​Simone holds a special interest in helping people cope with chronic pain and chronic health issues: Research is increasingly showing us how important the connection between mind and body is to the physical symptoms we experience - that our thoughts, feelings and actions play an important role. Simone conceptualises chronic pain and health issues in a multi-dimensional way - examining both the physical and emotional aspects and the ways in which we relate to and think about our physical symptoms. Evidence also shows us that a multidisciplinary approach to pain and chronic health treatment can lead to better outcomes. Based at PDC Health Hub, Simone works alongside our allied health team, including: Exercise Physiology, Physiotherapy, Diabetes Education, Pharmacy, Dietitian Services and Podiatry. This allows Simone to work collaboratively and provide shared care for her clients with Diabetes and other chronic health conditions. Because each person's experiences and challenges are unique, Simone utilises a flexible and integrative approach based on her training in: Psychodynamic Psychotherapy Schema Therapy Somatic Experiencing NeuroAffective Touch Attachment focused therapy Cognitive Behavioural Therapy (CBT) Dialectical Behaviour Therapy (DBT) Mindfulness based therapy Simone has also completed her Yoga (Hatha Vinyasa) Teacher Training, registered at 350hrs with Yoga Australia. ​ Simone is registered with the Australian Health Practitioner Regulation Agency and is a member of the Association of Counselling Psychologists.

Vanessa Tanner

Vanessa Tanner graduated from Murdoch University in 2021 with a degree in Exercise Physiology. Whilst completing her degree she worked within multiple allied health practices where she discovered her passions towards certain chronic diseases. Vanessa has experience working with NDIS clients, women's health, type 2 diabeties, and children. She looks forward to expanding her knowledge with us at PDC Health Hub, and is already proving to be a highly valued member of the team! Vanessa prides herself on providing clients with a holistic, well rounded approach to exercise that takes their lifestyle, as well as personal factors, into consideration. She understands the crucial role that setting realistic goals, and identifying barriers plays in ones health and fitness journey. This is something she continually provides support and guidance with during her sessions with clients.
