Our Mission is to close gaps for Determinants of Health in vulnerable populations (particularly the elderly impacted by poverty, language barriers, and low educational achievement) by increasing access to Diabetes Self-Management/ Training and Prevention Education in local communites and by engaging community stakeholders working together to make "healthy choice" the default choice for local community members... Our program in Diabetes Self-Management Education/ Training (DSME/T) will empower the diabetic and or caregiver with the necessary training, information and skills to effectively manage and control diabetes. The program is Nationally Accredited and covers 7 key components... We are a provider of the National Diabetes Prevention Program (NDPP). This is a Center for Disease Control and Prevention (CDC) led evidence-based program that facilitates participants in making simple lifestyle changes to prevent Type 2 diabetes.
