Gary Forman has been active in the field of long term care insurance for 29 years and currently serves as a senior vice president of Long Term Care Associates, Inc.
Prior to co-founding LTCA in 2002 Mr. Forman managed brokerage operations at Forman & Associates, Inc., a pioneering firm in the LTC Insurance field. He also holds the copyright to The Guide to Long Term Care Insurance for People with Medicare. While at LTCA, Mr. Forman's responsibilities have included the implementation of the company's remote sales and eMarketing platforms. Additionally, he manages the direct mail and online advertising programs.
Mr. Forman manages LTCA's affinity marketing program, liaisoning with over 40 alumni, credit union, professional, and fraternal organizations. He is also active in the management of day-to-day operations at LTCA.
Mr. Forman attended the University of Washington and resides with his wife and two children in Snoqualmie, Washington.

Rich's career of helping clients plan for long term care spans over a dozen years, beginning in his native state of Connecticut before branching out across the US. By 2012 his experience and excellent reputation had served to win him an invitation to join the team of elite specialists at LTCA, whose agents were honored by the American Association for Long-Term Care Insurance by ranking in the Top 1% of all LTCi producers nationwide (2010, 2011).
By virtue of his affiliation with LTCA, Rich has been able to capitalize on the advantages of multiple top-rated carriers and can uniquely tailor solutions to each of his clients' needs. As the endorsed provider of LTC insurance information to groups as varied as the American Society for Clinical Pathology and Tulane University Alumni Association, LTCA provides its agents with an opportunity to share their knowledge with concerned individuals from all over America.
Connecticut and National Partnership certified, Mr. Levine has also served as advisor to financial planners, elder law attorneys, and businesses. He remains proudest of his work with individual families, where he's been successful in helping remove the financial and emotional burden associated with a devastating long-term care event.
With his wife Judie, he has three children and five grandchildren. Rich has coached both soccer and baseball, and has been a member of the local hockey board. For the past several years, Rich has volunteered his time to the Alzheimer's Memory Walk, and the Cancer Association.

Based in Nashville, TN, Mr. Forman has spent the past 34 years focused in the long term care insurance market and currently serves as the president of Long Term Care Associates, Inc. With career beginnings as a long term care insurance agent he went on to build and manage a brokerage in excess of 3,000 producers.
Prior to joining LTCA in 2003, Mr. Forman rose to the position of president of Long Term Preferred Care (LTPC), formerly a subsidiary of Affinion Group and Cendant Corporation. LTPC was the largest independent marketer of long term care insurance in America with more than 200 affinity relationships including credit unions, alumni groups, and fraternal organizations. LTPC supported a sales force of over 300 career agents in addition to its brokerage.
Mr. Forman has participated in a variety of advisory roles to long term care insurers over the years including CNA, Allianz, and Genworth.
Like most successful companies, ours was founded to serve a growing need and was nurtured into success by the passion and dedication of our company founders. Incorporated in 1972 by Frank M. Forman, Forman & Associates, Inc. soon spread across 11 Western states as a pioneer in the long term care insurance field.
By the mid-1980's, Mr. Forman was courted by some of the largest insurers in the country to help design and develop their LTC insurance products to address this emerging trend.
By the time the "second generation" of LTC products came to market in the late 1980's, so too came the second generation of Formans into the now nationwide family business. Eldest son Robert Forman took over a national distributor based out of Nashville known as Long Term Preferred Care (LTPC) and grew it into the largest of its kind in the nation, capitalizing on increasingly consumer-driven policies now covering a vast array of care modalities such as Assisted Living and Home Health Care.
As the demographic trend known as the "Graying of America" took hold and mobility made care from remote family members less likely, Frank's other children Gary and Stephen responded to the growing hunger for agent training in the Pacific Northwest to meet the challenge of the burgeoning industry. Frank Forman remained a sought-after speaker for groups as diverse and distinguished as the Department of Health and Human Services, the American Health Care Association, the National Council on Aging, the National Association for Home Care, and the National Governors' Association, and provided expert testimony before the United States Congress on LTC-related issues.

Mr. Forman has spent the last 30 years helping producers and clients plan for their future long term care needs. Although agents from all across the country seek his expertise, he is proudest that his own LTCA producers rank in the top 1% of Specialists nationwide, as recognized by AALTCI, our industry's trade group.
