As the recipient of the National Resource Center for Patient/Family-Centered Medical Home (NRC-PFCMH), the American Academy of Pediatrics (AAP) is one of three organizations funded via this new cooperative agreement from the Maternal and Child Health Bureau (MCHB) of the Health Resources and Services Administration (HRSA). The goal of the cooperative agreement is to implement innovative programs and activities to address fragmented systems of care for children and youth with special health care needs (CYSHCN)... The AAP was founded in 1930 by 35 pediatricians to serve as an independent forum to address children's health needs. At that time, the idea that children had unique developmental and health needs was new. Practices that are now standard preventive care (i.e. immunization, regular health exams) were only just beginning to change the custom of treating children as "miniature adults"...
