We assist patients in achieving the best health and quality of life possible by preventing disease, stabilizing current chronic conditions, and preventing acceleration to higher-risk categories and higher associated costs. We focus on improving Quality of Life, improving functional status and avoiding unnecessary hospitalizations. We partner with health systems & health plans and collaborate with local providers to care for the At Most Risk (AMR) clients with multiple chronic disease diagnosis.
