Through BTG's relationships with community-based medical, behavioral and social service organizations, we are able to provide comprehensive care management services designed to help our clients to navigate the often complex medical, behavioral and social service systems... Bridging The Gap (BTG) Care Management Services' mission is to provide quality care management services to individuals who have felt the sting of a life of turmoil through sickness, mental illness, or family tragedy. It is our goal to partner with them so that they receive the right services, at the right time and in the right amount while raising them to a new level of self-sufficiency and determination in life.
