Launched in 2020 and gaining altitude, TRANSFORM HF is an Institutional Strategic Initiative created in joint support and partnership with the University of Toronto and the Ted Rogers Centre for Heart Research... TRANSFORM HF aims to build, support, and seed fund a community of engineers, scientists and clinicians who will work in tandem with Indigenous health experts and patients to develop point-of-care diagnostics, wearables, and AI technologies to monitor and proactively treat people with heart failure in their homes - and empower them to be more active in their own care... TRANSFORM Heart Failure seeks to address inequity in care using digital technologies and AI using patient-guided research utilizing the expertise at the University of Toronto.
