TRANSFORM

Launched in 2020 and gaining altitude, TRANSFORM HF is an Institutional Strategic Initiative created in joint support and partnership with the University of Toronto and the Ted Rogers Centre for Heart Research... TRANSFORM HF aims to build, support, and seed fund a community of engineers, scientists and clinicians who will work in tandem with Indigenous health experts and patients to develop point-of-care diagnostics, wearables, and AI technologies to monitor and proactively treat people with heart failure in their homes - and empower them to be more active in their own care... TRANSFORM HF and The Heart Hub want to build and sustain a vibrant community of local People With Lived Experience (PWLE) who are "research-ready!" This free, three-part workshop series was co-developed by PWLE with the goal of training and empowering patients, caregivers, and family members.
