Community Health Development Partners, LLC is dedicated to improving healthcare quality, accessibility, and affordability in low-income, underserved, and rural areas throughout the country. While these groups have a hard time receiving adequate primary care, receiving specialty care in their communities is almost impossible. That's where we come in... Much of an individual's health is influenced by the community around them. Our communities affect not only our health behaviors, but also the care we are able to receive and the cost we pay for it. In our communities, we work to address both these issues by providing outreach to the communities about health behaviors, seek input on how our communities want to receive care, and provide access to needed care that was unavailable before. By addressing all of these factors, we strive to improve health, one community at a time.
