CENTERED CARE

The Ohio Person-Centered Care Coalition was formed in 2005 by stakeholders in the Ohio long-term care industry. The coalition is comprised of a diverse group of organizations representing providers, consumers, and government agencies... After all, when does a person stop having hopes, dreams, and desires? Sometimes, challenges such as memory problems or serious illness impair our ability to communicate, but our simple pleasures are still there. They may be discovered through observation or a conversation where we take the time to really listen. For many elders, simple pleasures usually include activities like going to church, eating out on occasion, and visiting with family members... The mission of the Ohio Person-Centered Care Coalition is to influence and support transformational culture change in the long-term care environments where all individuals can experience meaning and purpose.
