At Denton Psychological Services we take your privacy very seriously. All clients are provided with a copy of the Notice of Privacy Practices. Anyone seeking services at Denton Psychological Services will need to complete the Consent Form, Acknowledgement of Receipt of Privacy Notice, and Background Information Form. You may complete them at your first appointment, or you may download and print them and bring the completed copies to your first appointment.
