VNA CONSORTIUM

VNA Consortium, Inc. is a collaborative, non-profit organization that uses an innovative approach to bring Boston hospitals and local Visiting Nurse Associations together. Founded in Boston in 1992, we are comprised of independent VNAs throughout Massachusetts and Rhode Island. We employ liaison nurses who are placed in Boston's prestigious hospitals to help case managers connect patients with quality home health care services in their communities, upon discharge.
