Forms & More is a small family owned and operated business providing exceptional service locally to Wisconsin, Upper Michigan, and Northern Minnesota. We have been supplying financial institutions and commercial businesses of all sizes the very best in printed forms, promotional products, and office products for more than 18 years. After 20 years in banking and 8 years working in the printed forms industry, David Fraker founded Forms & More in 2002 with a goal to provide creativity and an exceptional level of customer service. With the start of the company, he expanded from printed forms to promotional products and a complete line of office products. One year later, his daughter, Julie joined him in the business. When David retired in 2018, Julie's husband, Dave Pataconi joined the company to continue carrying on the sales and servicing of our valued customers.
