Dr. Bolhofner served as the Director of Orthopaedic Trauma Services at Bayfront Medcal Center from 1986-2016. Dr Bolhofner has remained dedicated to the orthopaedic surgical care of the severely injured throughout his career.

Clinton B. Davis

Dr. Davis is the former Chief of surgery at Edward White Hospital, having served as the Chief of the Division of Orthopaedic and Neurosurgery at Bayfront Medical Center for over 10 years. Dr. Davis is Board Certified in Orthopaedic Surgery with a sub-specialty in Spine Surgery. He is well known in the Tampa Bay area for his extensive experience in the treatment of complex spinal disorders. Dr. Davis retired from clinical practice in May of 2020 but is still an intricate part of our AFO family. He is focusing on his legal practice which includes Independent Medical Evaluations, Expert Medical Advisory, Compensatory Medical Evaluations and Medical Records reviews.

David B. Johnson

Dr. Johnson is a board eligible fellowship trained orthopedic surgeon specializing in minimally invasive, rapid recovery hip and knee replacements. He is a native to Florida being raised in Orlando and completed his undergraduate at the University of Florida. Dr. Johnson later attended medical school in Bradenton, FL where he graduated top of his class. He then completed his residency in Columbus, OH at Ohio University and Grant Medical Center where he was named chief resident and received the coveted Consultant of the Year award and Quality Champion award. He returned to his home state of Florida for his fellowship specializing in Adult Reconstruction at Florida Orthopaedic Institute. Through extensive training, Dr. Johnson has developed expertise in robotic surgery, muscle sparing direct anterior hip replacements, partial and total knee replacements, and the treatment of failed total joint replacements including infections. Dr. Johnson has been very active in research throughout his career, publishing many peer-reviewed articles and book chapters in orthopedics. He is a reviewer for the Journal of Arthroplasty reviewing the most up to date literature submitted for publication on joint replacement surgery. He is also a privately published author writing one of the leading books for early orthopedic education. Dr. Johnson is a member of the American Association of Orthopedic Surgeons (AAOS), the American Association of Hip and Knee Surgeons and the American Osteopathic Academy of Orthopedics (AOAO). Dr. Johnson takes a conservative first approach for the treatment of lower extremity arthritis. If surgery is ultimately required, he believes strongly in an individualized plan centered around each patient's specific needs. In his spare time, Dr. Johnson enjoys fishing, traveling, and spending time with his wife and children. He is an avid Gators fan and loves the Tampa Bay Lightning as well.

Dr. Benjamin D. Streufert

Patients with cervical, thoracic, and lumbar problems can require a range of treatments from the simple to the complex, and Dr. Streufert specializes in finding the strategies and treatment plans that best fit the unique needs of each patient. Specific interests include adult degenerative disorders, adult deformity, minimally invasive spine surgery, motion preservation surgery such as laminoplasty and disc replacement, and spinal trauma and tumor surgery. With emerging technology creating new options for patients with spinal disorders in both the surgical and non-surgical realms, Dr. Streufert has an interest in novel techniques that get patients better results. Dr. Streufert enjoys spending time with his family and taking advantage of all St. Petersburg has to offer.

Dr. Christian Mamczak

Dr. Christian Mamczak is a Orthopedic Traumatologist and fracture specialist. He is a Board Certified Orthopedic Surgeon by the American Osteopathic Board of Osteopathic Surgeons. Previous to joining All Florida Orthopaedics Dr. Mamczak held the position of Staff Orthopedic Traumatologist at Memorial Hospital Regional Trauma Center in South Bend IN. Dr. Mamczak is located at the All Florida Orthopaedic Associates office at Bayfront Hospital in St. Petersburg.

Dr. Jonathan Brown

Dr. Jonathan Brown, DPM, is a dedicated and accomplished podiatrist with a passion for serving others. His journey in healthcare began in the United States Army, where he served as a Combat Medic and Licensed Practical Nurse. After his military service, he pursued further education, earning a Bachelor of Science in Pre-Professional Biology and attended medical school at the California School of Podiatric Medicine Driven by a desire to continue making a difference, Dr. Brown completed a 36-month Residency Training program as a Podiatrist at the Department of Veterans Affairs in San Francisco. Throughout his career, he has served in various roles within the United States Navy, providing specialized podiatric care and enhancing medical services.

George H. Canizares

He received his Doctorate of Medicine degree from the University of South Florida College of Medicine in Tampa, FL. Dr. Canizares served his residency in Orthopaedic Surgery at Wayne State University School of Medicine in Detroit, MI. He completed a fellowship in Sports Medicine at South Florida Orthopaedic Institute in Coral Gables, FL. Dr. Canizares is Board Certified in Orthopaedic Surgery specializing in Sports Medicine. Dr. Canizares was selected to the NFL Neutral Physician Examiners Panel and served as the team doctor for the Miami Heat and Miami Sol. Dr. Canizares was instrumental in the development of the sports medicine program at All Florida Orthopaedic Associates and several Pinellas County high schools and colleges. He has also traveled with several professional sports teams as the team physician.

Glenn S. Fuoco

Glenn S. Fuoco, D.O., is board certified by both the American Board of Physical Medicine and Rehabilitation and the American Osteopathic Board of Physical Medicine and Rehabilitation with sub-specialty certification in Pain Medicine by both boards. Dr. Fuoco was born and raised in the Philadelphia area and completed his undergraduate education at Muhlenberg College in Allentown, Pa. He attended medical school at the Philadelphia College of Osteopathic Medicine, where he also interned. He completed residency in Physical Medicine and Rehabilitation at Baylor College of Medicine in Houston, Tx., and a one-year Interventional Spine/Pain Management fellowship at Florida Spine Institute in Clearwater, Fl. He has been in practice in the Tampa Bay area since 2002. Dr. Fuoco's philosophy on helping his patients with pain centers around identifying the primary pain generator and prescribing a comprehensive approach to improve a patient's pain and maximize functional outcomes. This can include a combination of physical therapy, interventional spinal procedures, appropriate prescription medications, as well as other modalities. A key philosophy to Dr. Fuoco's work is educating his patients on understanding why they have pain and why the treatment plan has been prescribed. He believes his patients should know as much as possible about their condition and that this understanding of the condition will ultimately help them maximize their outcomes during treatment.

Gregory I. Mines

Dr. Mines is board certified in both Sports Medicine and Family Medicine by the American Board of Family Medicine. He was born and raised in Clearwater, Florida where he participated in various sports including soccer, tennis, and golf. He earned his undergraduate degree from University of Miami prior to completing a Masters of Biomedical Sciences at Tufts Medical Center in Boston. He returned to the Tampa Bay area to attend medical school at the University of South Florida and a residency in Family Medicine at Bayfront Hospital in St Petersburg, Florida. Following the conclusion of residency, Dr Mines completed his fellowship in Primary Care Sports Medicine at the Steadman Hawkins Clinic of the Carolinas in Greenville, South Carolina, through Prisma Health. Dr. Mines works with athletes of varying levels ranging from elite to weekend warriors. He has served as team physician for the USA Karate National Team, traveling both domestically and internationally. He has also served as team physician for the Greenville Drive, a minor league affiliate of the Boston Red Sox. Additionally, Dr. Mines has served as team physician at the collegiate level with both Division I and Division II experience as well as for a multitude of local high schools. He has led medical coverage for mass events including triathlons (both open and closed water), half marathons, and large cycling events. Dr. Mines is a member of numerous professional societies including the American Medical Society for Sports Medicine, the American College of Sports Medicine, and the American Academy of Family Physicians. He has delivered numerous sports medicine lectures at the national, regional, and local levels. Dr. Mines cares for all musculoskeletal conditions in pediatric and adult populations. His experiences and patient relationships allow him to devise individually catered treatment plans that align with the goals and activity level of each patient. Utilizing the latest research and techniques, he practices evidence-based medicine to provide the highest level of care. Outside of the office, Dr Mines enjoys outdoor activities, including fishing and boating. He participates in a wide variety of sports and is an avid supporter of Tampa Bay's professional sports teams.

Jennifer M. Burns

Dr. Burns is board certified in Physical Medicine and Rehabilitation, Pain Management and has received advanced certifications in Interventional Pain Techniques. She is a Fellow in Interventional Pain Practice. Dr. Burns graduated in 1992 from the University of South Florida College of Medicine with a Doctorate of Medicine degree. Dr. Burns focuses on the "Whole Person". Her goal is to decrease pain, so the patient can exercise & return to activities. Dr. Burns will often teach core stabilization exercises in the office as a key component toward improving function. She offers a unique approach to Interventional Pain Management performing procedures such as Epidural Steroid & Facet Joint Injections, Radio-frequency Ablation and Sympathetic Nerve Blocks. She also specializes in Spinal Cord Simulator Placement.

Kurt C. Hirshorn

Dr. Kurt Hirshorn is a Board Certified and Fellowship Trained Joint Replacement Specialist. Dr. Hirshorn obtained his Masters Degree in Public Health from the University of South Florida College of Medicine in 2001 and, his Medical Degree in 2005. He completed his Orthopaedic training at Atlanta Medical Center. Dr. Hirshorn also completed additional specialty fellowship training in minimally-invasive hip and knee replacement surgery at RUSH University Medical Center in Chicago, IL. in 2011. Dr. Hirshorn obtained his Board Certification through the American Board of Orthopaedic Surgery in July 2013. Dr. Hirshorn has worked with the leaders in the field of joint replacement surgery, minimally-invasive surgery, implant design and testing. He has also worked as an assistant team physician for the Atlanta Falcons and Thrashers.

Michael J. Smith earned his medical degree from the University of Tennessee Center for Health Sciences. He did his internship and general surgery training at the John Hopkins Hospital in Baltimore, Maryland. He completed his orthoapedic training at the Campbell Clinic in Memphis, Tennessee. Dr. Smith is Board Certified in Orthopaedic Surgery. Dr. Smith has earned a well-deserved reputation as a leading specialist in Sports Medicine and Total Joint Replacement. He has been the team physician for many professional, collegiate and Olympic teams. He served as the team physician for the Tampa Bay Storm Arena Football League. He has been the orthopaedic consultant for the St. Louis Cardinals, Baltimore Orioles and the Cleveland Indians baseball teams. Dr. Smith was the Chief Physician for the Women's Olympic Basketball team at the Goodwill Games in Moscow, Russia, and for the US Women's Olympic Basketball Team during the Trials in Colorado Springs. Dr. Smith has published many articles on sports medicine and has been the past sports medicine editor of Orthopaedics Today. He has been on the publications committee of the American Orthopaedic Society of Sports Medicine as well as served on the council of Delegates Committee. Patrick J. Smith

Dr. Smith treats musculoskeletal conditions resulting from injury, overuse, and arthritis. He has a special interest in arthroscopic knee and shoulder surgery, knee and shoulder reconstruction, shoulder replacement surgery, and treating athletes and patients of all ages. He received his medical degree from the University of Tennessee Health Science Center in Memphis, TN. He then completed his Orthopaedic training at the University of South Alabama in Mobile, AL, followed by his fellowship training back in Memphis at the Campbell Clinic. Dr. Smith currently serves as a team Orthopaedic physician for the Tampa Bay Rays. He has previously been a team physician for the National Basketball Association's Memphis Grizzlies, University of Memphis, World Wrestling Entertainment, the NBA G-League's Memphis Hustle, and the Alliance of American Football's Memphis Express. Dr. Smith has published numerous articles in peer-reviewed journals and references with topics including reverse shoulder arthroplasty, knee meniscus surgery, labral tears in professional baseball players, as well as injuries in youth football players. Paul Pagano

Dr. Pagano is an Orthopaedic Spine specialist whose practice focuses on the diagnosis and treatment of all disorders of the cervical, thoracic and lumbar spine. Specific interests include adult deformity, minimally invasive spinal fusions, tumor resection and reconstruction, trauma and disc replacement surgery. Dr. Pagano is Board Certified in Orthopaedic Surgery with a sub-specialty in Spine Surgery. Dr. Pagano received his Bachelors of Science in Mechanical Engineering from the University of Cincinnati in 1991 and received his medical degree at the Ohio State University in 1995. Dr. Pagano completed his internship and orthopaedic residency at Wayne State University in Detroit, Michigan in 2000. Following his residency, Dr. Pagano performed a two-part Orthopaedic Spine fellowship with Dr. Richard Holt in Louisville, Kentucky and the late Dr. Thomas Whitecloud at the Tulane University School of Medicine in New Orleans, Louisiana. Dr. Pagano continues to use his engineering background as an active medical consultant holding patents in the field of spine surgery. He has also been an instructor in minimally invasive spinal surgery. Dr. Pagano is a native of the Youngstown, Ohio where he practiced for 13 years until relocating to the St. Petersburg area with his family in 2013.

Thomas S. Stang

Dr. Stang is fellowship trained in Orthopedic Trauma surgery and is board eligible by the American Osteopathic Board of Orthopedic Surgery. He was born in Osceola, Wisconsin and to this day is a die-hard Green Bay Packers fan and a shareholder. He enlisted in the Army reserves directly out of high school while pursuing his undergraduate degree from the University of Wisconsin. After September 11, 2001, he declined his commission and enlisted in the active-duty Army. He moved to Fort Bragg, North Carolina, where he was stationed for over 12 years as a Special Forces Medical Sergeant (18D) with the 3rd Special Forces Group (Airborne). During his tenure with the Special Forces, he served in 5 combat deployments and was honored with, most notably, the Purple Heart and two Bronze Stars, among many other awards and accolades. After serving our country for over 16 distinguished years, he was honorably discharged from the Army and moved to Tennessee, where he continued to pursue his love for medicine. He received his Doctorate of Osteopathic Medicine from Lincoln Memorial University in Harrogate, Tennessee. He completed his Orthopaedic Surgery residency at Louisiana State University Health Sciences Center in New Orleans. He then moved to Tampa, Florida, where he completed his Orthopaedic Trauma Fellowship at the Florida Orthopaedic Institute at Tampa General Hospital. Dr. Stang is a member of several organizations, including the Orthopaedic Trauma Association, the American Academy of Orthopaedic Surgeons, and the Florida Orthopaedic Society As an osteopathic orthopedic surgeon, Dr. Stang takes a holistic approach to patient care, recognizing that all body parts/systems are interconnected, and that the musculoskeletal system is fundamental to wellbeing and quality of life. Dr. Stang focuses on treating orthopedic injuries and fractures of all kinds, primarily due to trauma. He also has an elective orthopedic surgery practice focusing on hip and knee arthroplasty, including state-of-the-art joint replacement, deformity correction, and nonunion repair. In his spare time, Dr. Stang enjoys spending time with his wife, Lisa, and their two children, Nathan and Sophia. He enjoys activities such as biking, fishing, and hunting. Besides orthopedics and his family, his passion lies in woodworking and home remodeling.

Todd P. Beery

Dr. Beery earned his medical degree at Nova Southeastern University in Ft. Lauderdale, FL. He completed both his internship and residency at the University of Pennsylvania in Philadelphia, PA, where he served as Chief Resident. He pursued further training and completed a fellowship in Pain Medicine at the University of Maryland in Baltimore, MD. Dr. Todd P. Beery is triple Board-Certified in Physical Medicine & Rehabilitation, Pain Medicine and Electrodiagnostic Medicine. Dr. Beery uses a multidisciplinary approach to treat a wide variety of musculoskeletal and spinal disorders by incorporating physical therapy, medications and interventional procedures. His treatment philosophy is centered on delivering comprehensive, compassionate care with the goal of optimizing and restoring function. Treatment plans are individualized and integrate the use of minimally invasive non-surgical techniques such as fluoroscopic guided procedures, peripheral nerve blocks and ultrasound-guided injections. He also performs electrodiagnostic testing (EMG/Nerve Conduction Studies) to assist in diagnosing peripheral nerve injuries.
