ROOTS & ROADS COMMUNITY HOSPICE FOUNDATION

Roots & Roads Community Hospice Foundation was established as Casa de la Luz Foundation in 2000 as a 501(c)3 non-profit organization. In 2021, the Foundation changed its name to more fully encompass our community work and the reach of our programming. Roots & Roads is governed by an all-volunteer Board of Directors. Healthcare organizations supported by our mission include Casa de la Luz Hospice, Banner Health, Aria Hospice, Tucson Medical Center, Harmony Hospice and Heartland Hospice, among others... Roots & Roads Community Hospice Foundation is proud to be a sound steward of community gifts evidenced by over twenty years of fiscal responsibility. The Foundation is a 501(c)3 non-profit organization. Our Federal Tax ID Number is 86-1004321... Roots & Roads Community Hospice Foundation provides supplemental support for hospice patients and their loved ones; educates the community about end-of-life issues; and supports research projects to improve end-of-life care.
