Alex Meade - Chairman

Alex Meade is the General Manager and Principal Broker at the Miller Insurance Group in Salt Lake City. In his field, he holds a Certified Insurance Counselors Designation and is a member of the Society of CIC. Alex has worked with some of the oldest law firms, the largest independent auto dealerships, engineering firms, software technology and biotech companies in Utah. He works with businesses and individuals in most states west of the Rockies, in all areas of insurance and risk management, giving him a unique view for business. Alex holds a Bachelor of Arts Degree in Finance from Linfield College in McMinneville, Oregon. His community involvement includes being a Young Leader with the United Way of Utah, and a member of Fourth Street Clinic's Corporate Council helping to develop and start the Fourth Street Film Festival held annually in August.

Blake Fessler

Blake received his Master of Healthcare Administration from The University of Utah, David Eccles School of Business. Blake holds bachelor's degrees in foreign language and literature (Spanish) and nursing from Weber State University where he graduated Summa Cum Laude and was awarded a Crystal Crest for community service. Prior to starting his administrative career, Blake consulted for several successful start-ups in northern Utah. Blake worked for many years at McKay Dee Hospital in the nursing float pool, where he trained and worked in eleven sub-specialty areas. He moved to the emergency department where he was a charge nurse. He later worked in outpatient care in the PACU at Mountain West Surgical Center. Blake has a passion for serving vulnerable populations and has worked with various nonprofits including Habitat for Humanity, The Utah Special Olympics and Business for Health Solutions. He loves spending time with his wife and children boating, cycling and enjoying time together.

Christina Lau

Christina joins the Fourth Street Clinic Board of Directors having served on the boards of YWCA Utah, Voices for Utah Children, and Rowland Hall. She is the current Chair of the Advancement Committee. Her professional background is in marketing and brand strategy and she is an Account Director at the advertising agency, R&R Partners. Christina graduated from Wellesley College and earned an Ed.M. from Harvard in Human Development & Psychology. She lives in the Avenues with her family.

Dan Zuhlke

Dan Zuhlke is a retired healthcare executive with forty years of experience working as a Human Resources professional. He has worked as the Chief Human Resources Officer for several large Health systems, including The Detroit Medical Center, St. John Health, and recently retired from Intermountain Health. For the past fifteen years he was the Chief HR Officer for Intermountain Health, and the senior executive responsible for organizational alignment. His expertise includes executive compensation, leadership development, employee engagement, and organizational effectiveness and alignment. Dan received his Bachelors degree from Michigan State University and Master's degree from Wayne State University. Dan and his wife Sally have two grown daughters who live in New Jersey and Massachusetts.

Dave Jones

David Leta

David Leta is a financial problem solver. Between 1976 and 2022, he was a private attorney in Salt Lake City, Utah where his practice focused on commercial bankruptcy matters. During his career, he represented debtors, creditors, trustees, creditor committees, asset purchasers, vendors, equity holders and other parties in all aspects of financial disputes, including collections, foreclosures, workouts, receiverships and bankruptcy. Between 1992 and 2018, he was a partner at Snell & Wilmer LLP, a regional law firm and was the head of its Salt Lake City bankruptcy group. In January 2018, David became Of Counsel to Snell & Wilmer and on December 31, 2022, he retired from the active practice of law. David's experience enables him to analyze a financial situation from multiple perspectives and design creative, durable solutions. As an advocate, he also previously handled litigation for clients involving the United States Bankruptcy Code, the Federal and Bankruptcy Rules of Procedure, the Uniform Commercial Code and applicable state laws. David now does business as "Leta Bankruptcy Mediations" where he serves as an independent mediator of financial disputes for non-clients.

Dr. Laura (Eli) Moreno

Dr. Laura (Eli) Moreno is a Family Medicine physician. She practices full spectrum family medicine with obstetrics at the University of Utah. Dr. Moreno earned a medical degree from the Universidad de Guadalajara in Guadalajara Jalisco, Mexico. She completed a surgical internship at Northwestern University in Chicago, IL and family medicine residency at Valley Family Medicine in Renton, WA. She is one of the faculty for the residency program at the University of Utah and has cultivated her career in academics through scholars programs at the University of Washington in Seattle, WA and Georgetown in Washington D.C. Her outpatient clinical practice is at the Redwood Health Care Center in the University of Utah, which she joined in 2019. Her passions are serving the Hispanic and Refugee population in Salt Lake City and teaching residents and medical students the art of medicine with a focus on health equity, diversity and inclusion, cultural competency and woman's health.

France A. Davis II

France A. Davis II is the founder of Mental Health Experience, L.L.C. France is a board-certified, Utah licensed Physician Assistant who specializes in Psychiatry. He obtained his Bachelor of Arts degree in Sociology and Theology from Georgetown University and his Master of Science in Clinical Medicine / Physician Assistant Studies from Marywood University. France spent 4 years in Orthopedic Surgery and has been practicing Psychiatry for the past 9 years. France sees a broad spectrum of mental health conditions including ADHD, Depression, Anxiety, Bipolar Disorder, Schizophrenia, OCD, PTSD, Sleep Disorders, and more, with a special interest in reproductive mental health issues experienced by mothers and their partners. France also has a special interest in the treatment of athletes dealing with mental illness. France feels a special connection to this subspecialty given his extensive athletic background. France is part of the Advanced Practice Clinician team for the University of Utah Department of Psychiatry and Huntsman Mental Health Institute Crisis Services. George Durham

Dr. Durham is an Adjunct Professor Emeritus in the Department of Pediatrics at the University of Utah School of Medicine - having served on the Admissions Committee since 2014. He holds a BA in biochemical sciences from Harvard College and graduated from Duke University's School of Medicine in 1973, completing his residency in pediatrics at the University of Utah in 1976. He practiced with Bryner Pediatrics from 1976 - 2014 and is a past medical staff president, chair of Quality Improvement and former member of the Governing Board of Primary Children's Hospital. He was a pediatric volunteer physician at the Fourth Street Clinic in the mid-90s and has served on Fourth Street Clinic's board since 2012. In 2017, he was appointed to utah's Juvenile Justice Services Board.

Glen Lambert

Glen is the former Executive Director of Odyssey House of Utah, a substance abuse and social service agency, a position he held for over 30 years. During his tenure, Odyssey House grew from one program with 27 clients to a multi-site agency that serves up to 400 clients daily and a staff of 160. Glen is widely respected as an expert in the substance abuse treatment community and has conducted hundreds of consultations, workshops, classes and speeches in 23 states and seven countries. Glen holds a Master's of Science in Social Work from the University of Utah. In addition, he has been an active community steward serving on the Community Nursing Services Board of Directors, the Governor's Commission on Criminal and Juvenile Justice, the State Advisory Sexual Abuse Committee and more. He has been honored with the Governor's Award, Utah from the Substance Abuse & Anti Violence Council as well Clyde and Marie Gooderham Lecture Award from the University of Utah.

Jeanne DePaulis

Jeanne DePaulis spent her entire career in healthcare, almost exclusively at Primary Children's Hospital. She gained extensive clinical experience in the Outpatient Clinic, Behavioral Health Unit and as Nursing Director of the Infant Unit. She moved into administration and eventually retired as the Assistant Administrator responsible for Pharmacy, Respiratory Care, Family Support Services (social Work and Psychology, Child Life, Palliative Care, Chaplaincy, Language Services, Music Therapy, Patient Education), Staff Education, Safe and Healthy Families (Child Protection Team) and Primary Children's Behavioral Health Programs (inpatient, outpatient, residential). She also held administrative positions in both the Research and Ethics Committees. She holds a BA in Sociology from Wayne State University, a BS in Nursing from the University of Utah and a Healthcare MBA from Regis University as well as a Certificate in Conflict Resolution from the University of Utah.

Jeniece Olsen - COO

Jeniece Olsen is a Licensed Clinical Social Worker with two decades of experience in housing and homelessness, both in the US and in Australia. She received a master's in Social Work from the University of Utah and a master's in International Relations from the University of Queensland in Australia, where she was a Rotary World Peace Fellow. While living in Brisbane, Jeniece had the opportunity to work closely with refugees from Iraq, Iran, and Afghanistan on housing and behavioral healthcare. She also worked at Australian Red Cross, where she had the opportunity to lead a housing program for young people experiencing homelessness, as well as work in disaster response as an Evacuation Center Coordinator and a leader in Community Recovery during the 2011 floods. For over a decade, Jeniece worked at The Road Home in Salt Lake City, providing and overseeing affordable housing and stabilization services to individuals and families experiencing homelessness. During her time there, she was involved in affordable housing advocacy and has a deep understanding of social determinants of health as they relate to recovery from homelessness. Jeniece currently serves as the Chief Operating Officer at Fourth Street Clinic, overseeing both central clinic operations and outreach services at multiple locations across Salt Lake County.

Jim Ruble

Jim is assistant dean for student affairs and an associate professor (clinical) at the University of Utah College of Pharmacy. He is also a faculty relations specialist in the Office of Academic Affairs for the Senior Vice President for Health Sciences. Jim has adjunct appointments in the School of Medicine-Center for Health Ethics, Arts and Humanities, in the College of Law, and in Pharmaceutical Chemistry and Pharmaceutics at the University of Utah. His teaching, scholarship and clinical activities focus on pharmacy and healthcare law, ethical dilemmas in healthcare, FDA law and regulation, health care policy, pharmacy compounding, community pharmacy practice, communication and conflict. Jim is a member of the Utah Bar and the US Patent and Trademark Office Bar. Jim is trained in conflict resolution and is a mediator. He received academic degrees in science (BS Biology), pharmacy (BS Pharmacy and PharmD), and law (JD) from the University of Utah.

Karen Shepherd

Karen Shepherd is a Former Member of Congress and Utah Senator who now serves on several corporate and community boards. She has had a long and varied career that began when she taught Freshman English and literature at The American University in Cairo and at BYU. She then served as Assistant Director and later as the first women Director of Salt Lake County Social Services. She was then Director of Continuing Education at Westminster College. The best job of her life was when she was the editor and publisher of Network magazine and the owner of Webster Publishing. The last job of her life was when President Clinton appointed her to be the U.S. Representative at the European Bank for Reconstruction and Development (EBRD) in London. Her many years of community work have focused on organizations that promote the health and safety of women and children and on campaign finance reform.

Kelley Lange

Kelley has been Vice President of Operations at High Country Line Construction since 2015. In this role, he is responsible for customer relations, safety, estimating, project management, profitability, and overall successful project delivery. In his nearly 20 years of construction experience, Kelley has gained valuable construction project expertise in hospitals, pharmaceutical plants, renewable energy, electrical grid and EPC/design build delivery. Prior to High Country, Kelley spent four years with Atkinson Power as project manager and four years with parent company, Clark Construction as assistant superintendent and project engineer. He holds a Master of Business Administration from the University of Denver, and a Bachelor of Science, in civil engineering from the University of Maryland, College Park. Kelley is an active member of NECA Southwest and Missouri Valley chapters and a board member of the Fourth Street Clinic - supporting future growth with the programs and partnerships committee.

Kim Wirthlin

Laurel Ingham

Laurel was appointed Development Director in February 2014. She brings to Fourth Street Clinic over 30 years of development experience having worked at the University of Utah, Valley Mental Health, Utah Symphony, Utahns for Choice, and several political campaigns. She has managed all aspects of fundraising from grant writing, special events to donor cultivation and stewardship working with individuals, corporations, and foundations. Laurel holds her B.S. in Community Health Education from the University of Utah.

Libby Ellis - Chairman

In her professional life, Libby has raised funds for The Nature Conservancy around the globe, opened Salt Lake City's first public recycling center, worked as an NPS Backcountry Ranger at the Grand Canyon, directed Patagonia's environmental program, and more. Currently, Libby serves as the Senior Director of Development at the Grand Canyon Trust. She wholeheartedly believes in community engagement and giving back. Maria Hill

Maria Hill is the Assistant Vice President, Enterprise Care Management for Intermountain Healthcare. Her primary responsibility in this role is oversight of care management functions across all divisions including Hospitals, Medical Group, and SelectHealth. Maria has more than 30 years' experience with corporate, academic, and community-based health systems, where she was instrumental in the development and implementation of care management methodologies. Her work in the continuum of health care service is focused on delivering the best practice and results in value-based payment arrangements including Bundled Payment, Accountable Care Organizations and Medicare Advantage Plans. She is trained as a Clinical Nurse Specialist in Chronic Care. Her work in Lean Design and 6 Sigma assists her in achieving excellent results in quality, cost of care and service excellence. Marilynn Paine

Marilynn Paine served previously as the Assistant Vice President for Health Sciences Strategic Initiatives at the University of Utah. In this position, she was involved in or led several initiatives that span traditional academic units and employ tools around systems thinking. These include ambulatory care process redesign and improving the patient experiences in the clinical enterprise. In addition she worked on the governance process that led to the establishment of the University of Utah School of Dentistry, and helped shepherd through governance the new Population Health Sciences Department in the School of Medicine. Before coming to Utah, she was at the University of Michigan for 19 years where she was an Assistant Vice President for Medical Affairs and Chief of Staff to the Executive Vice President for Medical Affairs. She helped to establish and participate in leadership development initiatives for the Health System, and she is a certified leadership coach. Mason S. Turner

Dr. Mason Turner is the Associate Chief Medical Officer for Behavioral Health for Intermountain Health based in Salt Lake City, UT and Denver, CO. His leadership in mental health care focuses on the development and evolution of strategic innovations in behavioral health care delivery that enhance quality of care and clinical excellence and are guided by principles of equity and inclusion of the patient/consumer voice into all aspects of the behavioral health ecosystem. Including integration initiatives which link mental health care and addiction medicine services with both primary and specialty medical care, as well as supporting innovative technological means of reaching patients for treatment of behavioral health conditions, he is also committed to enhancing access for behavioral health care services to all those who need it. Additionally, he has extensive experience in promoting employee health and well-being in a variety of corporate and healthcare sectors. As a clinician, he specializes in addiction medicine, consult-liaison psychiatry and treatment of personality disorders. He has received extensive training in resiliency-and systems-based approaches for improving the mental health and well-being of refugees, asylum seekers and torture survivors. Dr. Turner also served as a former Assistant Clinical Professor in Psychiatry at the University of California, San Francisco Medical School, where he taught medical students at all levels the practice of interviewing patients and clinical reasoning skills. He also served as adjunct faculty at the University of Hawaii John A. Burns School of Medicine. Dr. Turner earned his AB degree in psychology from Dartmouth College in Hanover, NH, and completed his MD degree at the University of Texas, Southwestern Medical Center/ Parkland Hospital in Dallas, TX. He completed his residency in psychiatry at Harvard University Medical School through Massachusetts General Hospital and McLean Hospital, in Boston, MA, with additional training in psychotherapy at the Center for Psychoanalytic Studies at Massachusetts General Hospital and addiction psychiatry at McLean Hospital. Meghan Fry

Meghan is a Clinical Mental Health Counselor who has served the underrepresented populations of Salt Lake County for nearly 25 years. She spent the first two decades of her career with Volunteers of America, Utah (VOA). Meghan held several different roles at VOA, including Director of the Center for Women & Children, a social detoxification center. She also managed the Assertive Community Outreach Team, a wrap-around team that focused their efforts on the chronically homeless individuals of Salt Lake County. After several years in social services, she received her master's degree in Mental Health Counseling from the University of Phoenix. After completing her degree, Meghan worked in VOA's Cornerstone Counseling Center on the Adult Substance Abuse Team, eventually serving as the Division Director of Treatment Services. The Division Director position gave her the opportunity to oversee all operations of both Cornerstone and Family Counseling Centers.

Melissa Mason

Melissa Mason received her BSN from the University of Utah in 2005 with an emphasis in geriatrics. She joined Fourth Street Clinic in 2014 bringing with her 12 years of hospital and ICU experience. Since joining the clinic, she has held a number of roles including Diabetic Educator, Clinic Manager, and most recently Nursing Director. In addition to supervising the nursing team at the clinics, she oversees the Medical Assistants, Lab Coordinator, Clinic Scheduling, COVID-19 testing team and vaccine team, and volunteer nurses. Melissa also is a member of the Senior Management Team.

Rebecca Schneider

Following residency, Dr. Schneider joined the dental team at Fourth Street Clinic in July 2021. Dr. Schneider understands a visit to the dentist can be anxiety-inducing. For that reason, Dr. Schneider and her dental team do everything they can to provide a friendly, relaxing, and non-judgmental environment to reduce anxiety and reverse the stigma associated with a dental appointment and make it a positive experience. Their dental team has a motto at Fourth Street Clinic, "While you're here, you are family." Dr. Schneider and her dental team are committed to advocating and improving the oral health of all their patients regardless of their insurance status and the ability to pay. Dr. Schneider is excited to be in a profession that combines art, and science while improving the confidence, health, and comfort of her patients. Dr. Schneider welcomes all new and existing patients to stop and visit the dental clinic.

Tiffiny Gregory

Prior to joining Fourth Street Clinic in 2012, Tiffiny worked at the Utah State Hospital, an inpatient lock-down mental health hospital for chronically and severely mentally ill adults, youth and children, focusing on getting institutionalized adults discharge ready for community placement and as well as setting up youth and child sensory integration programs to reduce trigger behaviors. Tiffiny pursued a degree in Physician Assistant studies at the University of Utah, which provided a variety of opportunities to work in rural and medical resource-limited areas in Utah. Since starting at Fourth Street Clinic, she has had many roles in working with chronically and medically fragile homeless patients. As the Clinic Manager she assisted in implementing women's health day for screening mammogram services for uninsured women between 40-64 years old. In her current position as the Clinical Operations Director, she coordinates and collaborates with the medical director and chief executive officer to maintain and ensure compliance measures for HRSA grants. In addition, she oversees the quality improvement activities as well as staff training.

Van Aston

Van Aston grew up bouncing around the country with his Air Force family who eventually settled in Ogden, Utah. He earned his Bachelor of Science in Clinical Laboratory Sciences from Weber State University in Ogden. He continued his education in Atlanta, Georgia where he attended Emory University and graduated in 2011 with a Master of Medical Science degree as a Physician Assistant. At Emory, he was awarded the prestigious Emory PA Heritage Award which is given to the student that best represents the mission of the program. Van began his PA career in South Georgia working in a small community health center. He remained there for two years until the call of the mountains and home was too strong to resist. He moved back to Ogden and continued his work in community health in the town where he spent his most formative years. Van began working with people experiencing homelessness in 2015 at the Hope Clinic located inside the Lantern House Shelter in Ogden. Together with the clinic director, he worked to build the clinic and its community impact. In 2022, Van joined the team at Fourth Street Clinic as the Medical Outreach Street Team (MOST) provider. In October 2023, he was named the Medical Director.

Vy Bui

Vy received his PharmD degree from D'Youville University, College of Pharmacy in Buffalo, NY. In 2011 he began his career with Walgreens as a Pharmacy Intern. Upon graduating in 2014 he relocated to Salt Lake City, Utah and worked as a Staff Pharmacist with Walgreens and was quickly promoted to Pharmacy Manager. During his tenure, he has guided the pharmacy through Salt Lake's population growth as well as through COVID-19. His work was integral in mitigating the community spread of COVID-19 through testing and vaccinations both onsite and at local nursing homes. Additionally, as a representative for Walgreens, he partnered with the Salt Lake County Health Department to dispense tuberculosis medications for newly arriving immigrants and refugees. He was also instrumental in conducting yearly vaccination clinics at local community centers and private employers. Vy joined Fourth Street Clinic in July 2022. He is excited to bring his experiences in providing medications, vaccinations, and education to the disadvantaged community.
