Alicia Stafford

Alicia Stafford has worked as a nurse since 2004, when she started her career as an LPN. She continued her education through Excelsior College and obtained her RN license in 2010, and most recently completed a newly available program through The Ohio State University and received her Registered Nurse in Primary Care Certificate (RNPCC). In addition to her experience in primary care, Alicia has worked in the home health and long-term care setting. A true "hometown girl," Alicia grew up in Logan County and graduated from Benjamin Logan High School.

Alyssa A. Kauffman

Alyssa Kauffman is no stranger to Logan County, as she grew up in West Liberty and graduated from West Liberty-Salem Local Schools. After high school, Alyssa attended Bluffton University in Bluffton, Ohio, where she earned a degree in Food and Nutrition with a concentration in Diatetics and a minor in Sociology. During her undergraduate years, she had the opportunity to be involved in various hands-on settings, including clinical experiences at St. Rita's Hospital in Lima, Ohio, providing a Harvest Meal for the community and participating in Wellness Fairs. Upon graduating from Bluffton in 2017, Alyssa moved to Brookings, South Dakota. There, she attended South Dakota State University (SDSU) for a master's degree in Nutrition and Exercise Science. Within this graduate program, she also completed the required supervised internship hours to sit for the national registered dietitian examination. During these programs, she focused on research for her thesis project concerning the older adult population. Specifically, she focused on how nutrition and dietary intake affect muscle and overall body composition and health in that population. During the internship portion, she had various experiences, including clinical and food service rotations at the local rural hospital, WIC, local schools, Native American Reservations, wellness centers and local grocery stores. These experiences contributed to Alyssa's interests in community nutrition and population health, as well as working with all ages, but she enjoys working with the younger generations and has a certificate in Transdisciplinary Childhood Obesity Prevention, as well, from SDSU. Outside of work, Alyssa enjoys spending time with family and friends, exploring new restaurants, traveling, being active with yoga, Zumba, walking, or rollerblading, reading or putting together puzzles.

Andy Stahl

Andy Stahl is a Licensed Professional Clinical Counselor Supervisor who is Board Certified through the State of Ohio Counselor, Social Worker, Marriage & Family Counseling Board. He received both his Bachelor of Arts in Social Science Education and his Master of Science in Clinical Mental Health Counseling from Wright State University in Dayton, Ohio. Prior to joining Community Health & Wellness Partners, Andy began his professional career at Daybreak, Inc, in Dayton, Ohio, where he worked with homeless youth who frequently had mental health and substance abuse concerns. Andy enjoys collaborating with patient's primary care providers to treat conditions such as anxiety, depression, Attention-Deficit/Hyperactivity Disorder, and Post-traumatic Stress Disorder. A Benjamin Logan High School graduate, Andy is originally from Belle Center, Ohio, and currently lives in rural Logan County with his wife and four children. In his free time, he enjoys spending time with his family, traveling, and watching and playing sports with his children.

Angie Mahaffey

Angie Mahaffey is certified as a Family Nurse Practitioner through the American Association of Nurse Practitioners. She earned an Associate Degree of Applied Science in Nursing from Marion Technical College in 1998. In 2013 she earned a Bachelor of Science degree of Nursing from Chamberlain College. In 2018 she earned a Master of Science in Nursing from West Virginia Wesleyan College. Angie's background as a nurse has mostly been in the emergency department. Angie was a full-time Assistant Nursing Professor at West Virginia Wesleyan College and taught undergraduate Pediatrics and Obstetrics for the BSN nursing program while working as a nurse Practitioner in Urgent Care and Primary Care. Angie is passionate about health promotion and disease prevention, as well as acute and chronic disease management. Angie is originally from Marion and now lives in Bellefontaine with her husband and son. She enjoys reading, knitting, hiking, yoga, and spending time with her family.

Ashley Brewer

Ashley Brewer is a Licensed Social Worker who received degrees in social work and psychology in 2009 from Wilmington College. Ashley has spent most of her career working in Logan County with children and adults with behavioral health needs. Most of that time was spent at a local agency helping at risk youth at Adriel Inc., where she provided oversight of two of their group homes, and at the Logan County Board of Developmental Disabilities as a Service and Support Administrator. In October 2017, Ashley joined Community Health and Wellness Partners as the Behavioral Health Coordinator. Ashley has spent her time here developing the Behavioral Health Program and now provides program oversight while assisting patients with their various behavioral health and social needs. Ashley lives in Bellefontaine with her dog Mo. She enjoys traveling, reading, coaching local sports teams and spending time with friends and family.

Bethany Henry

Bethany Henry is a Licensed Professional Counselor who is Board Certified through the Counselor, Social Worker, Marriage & Family Therapist Board of Ohio. She received her Bachelor of Psychology from Grace College in Winona Lake, Indiana and Master of Professional Counseling from Grand Canyon University in Phoenix, Arizona. Prior to obtaining her master's degree, Bethany worked as coordinator of children's ministry at Grace Chapel. Bethany has worked at TCN Behavioral Health as a case manager and completed her professional counseling internship. Bethany is excited to be a part of the team at Community Health & Wellness Partners. She enjoys working with kids and adults, working alongside patients and helping meet various needs together. She grew up in Bellefontaine, where she now lives with her husband and two kids. She enjoys spending time with family, working out, reading, and actively participating within her community and church.

Breanna Detrick

Breanna Detrick is a certified Family Nurse Practitioner, certified through the American Association of Nurse Practitioners. She received her Bachelor of Science in Biological Sciences from the University of Toledo and her Master of Science in Nursing from The Ohio State University. Prior to becoming a nurse practitioner, she was involved in clinical research at Nationwide Children's Hospital and the Ohio State Wexner Medical Center. Breanna spent multiple student rotations at Community Health & Wellness Partners and has been serving patients of this community ever since. She welcomes patients of all ages and enjoys being able to take care of the entire family. Her focus is on health promotion and disease prevention, as well as acute and chronic disease management. Breanna and her husband, Jeff, currently live in Westerville. They are on a weekly bowling league and love to explore the national parks.

Dr. Charles Kratz

Dr. Charles Kratz joined Community Health & Wellness Partners on March 1, 2016, after caring for patients at Oakhill Medical Associates for 20 years. Dr. Kratz has served as the President of the Medical Staff at Mary Rutan Hospital and is currently the Medical Director for Hospice Services with Universal Home Health and Hospice. A Medical Degree recipient from Temple University School of Medicine, Dr. Kratz also attended Eastern Mennonite University where he received a Bachelor of Science in Biology with minors in English and Chemistry. He went on to complete his residency at Williamsport Hospital Family Practice Residency Program Williamsport, Pennsylvania. A member of the American Academy of Family Practice, Christian Medical & Dental and the Mennonite Medical Associations, Dr. Kratz has been Board Certified since 1996. He is married to Patricia Kratz and they have five children, and they live in rural Logan County.

Dr. Jillian Shellabarger attended Otterbein University where she received a bachelor's degree in Life Science with a minor in Sociology and graduated with honors in 2010. She then attended Wright State University Boonshoft School of Medicine and earned a medical degree in 2010. After completing a residency in psychiatry through Wright State University in Dayton, Ohio, in 2014, she moved to a fellowship in child and adolescent psychiatry through Wright State University in Dayton, Ohio, in 2015, serving as Chief Resident. She maintains board certification in both adult and child and adolescent psychiatry, and she is a member of the American Psychiatric Association and the Ohio Psychiatric Physicians Association. Born and raised in rural Ohio, she remains committed to providing psychiatric care to rural populations, and she has joined Community Health & Wellness Partners to broaden access to psychiatric care.

Dr. Kenneth Miller

Dr. Kenneth Miller joined Community Health & Wellness Partners on March 1, 2016, after caring for patients at Oakhill Medical Associates for nearly 28 years. Dr. Miller has served as the President of the Medical Staff at Mary Rutan Hospital and is currently a member of the Board of Directors of the Medical Group of Ohio. A Medical Degree recipient from The Ohio State University College of Medicine, Dr. Miller also attended Goshen College, where he received a Bachelor of Science in Biology. He went on to complete his residency at Grant Family Medicine Residency Program in Columbus, Ohio. A member of the American Academy of Family Practice, Christian & Dental and the Medical Mennonite Associations, Dr. Miller has been Board Certified since 1988. Dr. Miller and his wife, Doris, live in Bellefontaine and have three grown daughters.

Dr. Steve Toney

Dr. Steve Toney is joining Community Health and Wellness Partners in June of 2022 after practicing 26 years in rural West Virginia. Dr. Toney earned both his bachelor's degree in Chemistry and Medical Degree from West Virginia University. He went on to complete his residency at the Kanawha Valley Family Medicine Program at The Charleston Area Medical Center, serving as chief resident in his final year. In 2014, Dr. Toney received an MBA in Healthcare from George Washington University. He has been board certified by the American Board of Family Practice since 1996. Dr. Toney and his wife, Christine have four grown children, enjoy backpacking, cooking and playing games with family and friends.

Emily Pool is a Licensed Independent Social Worker Supervisor who is Board Certified through the Counselor, Social Worker, Marriage & Family Therapist Board of Ohio. She received her Bachelor of Arts degree from Wright State University in Dayton, Ohio, and a Master of Social Work (MSW) from The Ohio State University in Columbus, Ohio. Emily worked at Logan County Children's Services from 1994 until 2014, when she took a position with Consolidated Care Inc., as an Outpatient Therapist. She worked with adults and youth in the outpatient setting as well as providing services at Benjamin Logan Local Schools. In 2018, Emily was hired by Benjamin Logan as a School Social Worker. During this time, Emily also earned her Pupil Services License through the Ohio Department of Education. Emily is excited to be a part of the team at Community Health and Wellness Partners and to have the opportunity to continue to provide services to the students at Benjamin Logan Schools and their families in the school-based clinic. In collaboration with primary care providers, she enjoys working with patients who present symptoms of anxiety, depression, trauma and ADHD. Emily is a Bellefontaine native. It was important to her to return to the area after college and be able to serve her community. When not at work, Emily enjoys spending time with family and supporting her children in their activities.

Erin Dye

Erin has a history of working with teens and young adults with traumatic experiences, starting her professional career at Adriel, Inc., as part of the clinical department for residential services. Erin has professional experiences in inpatient, IOP, crisis response, and most recently school-based mental health services. Erin has a passion to work with people in all stages of life, and she enjoys serving as part of the integrated health care team at Community Health & Wellness Partners, working collectively alongside patients and helping meet various needs together. Erin is from West Liberty and continues to live there with her family. She loves to do anything that involves the outdoors, specifically walking or hiking, catching up with her friends and family, scrapbooking, and baking.

Hannah Auck

Prior to obtaining her master's degree, Hannah worked as a Social Services Director between two skilled nursing facilities near her hometown. Hannah has worked at Consolidated Care Inc., as both a home-based service provider and as a youth and family therapist providing services in both an outpatient setting and in area schools. Hannah is excited to be a part of the team at Community Health & Wellness Partners. In collaboration with the primary care providers, she enjoys working with youth and adults who present symptoms of anxiety, depression, trauma, and ADHD. Hannah is originally from a rural community in Crawford County, Ohio, and now lives in Union County. Hannah enjoys crocheting, gardening and baking. When not at work, Hannah spends as much time as possible with her family, friends, and church family.

Jason Martinez

Jason Martinez is originally from Pittsburgh, Pennsylvania, but moved to Ohio to attend Ohio Northern University, where he earned a Doctor of Pharmacy in 2009. Jason is a Certified Ambulatory Care Pharmacist through the Board of Pharmacy Specialties. Experienced in the retail and hospital sector with an interest in chronic disease management, Jason works collaboratively with our other providers to optimize treatment plans in addition to helping patients understand how to take their medications or find more affordable options. Jason lives in Union County with his wife and two children, and he enjoys sports and spending time outdoors.

Jodi Kiessling

Jodi Kiessling, CNP, is a board-certified family nurse practitioner through the American Academy of Nurse Practitioners. She earned her Associate Degree of Science in Nursing from Fayetteville Technical Community College in 2006, Bachelor of Science in Nursing from Wright State University in 2008, and Master degree of Science in Nursing from Otterbein University in 2021. Before becoming a nurse practitioner, she spent 15 years as a registered nurse in a variety of settings: critical care, labor and delivery, and the emergency department. She became a family nurse practitioner so she could provide holistic care to patients of all ages. During her training she spent two clinical rotations at Community Health & Wellness Partners as a student nurse practitioner. Jodi is very familiar with Logan County because she grew up in West Liberty and graduated from West Liberty-Salem High School. She currently resides in Delaware with her husband, Chuck, two sons, and two cats. In her free time, she enjoys spending time with family, reading, and gardening.

Joshua Bryant

Joshua Bryant is a certified Family Nurse Practitioner certified through the American Academy of Nurse Practitioners. He received a bachelor's degree in Biology and a master's degree in Entomology prior to earning a Master of Science in Nursing, all from The Ohio State University. Prior to becoming a nurse practitioner, Joshua spent five years working as an entomologist at Ohio State University, where he studied the effects of pesticides on household and structural pests. Once he obtained his RN license, Joshua worked as a staff nurse on a medical-surgical unit at The Ohio State University Wexner Medical Center in Columbus. Joshua completed a student rotation at Community Health & Wellness Partners and has a strong interest rural healthcare, including health promotion, disease prevention and management of chronic health conditions. Joshua grew up in a Pike County, Ohio, in Piketon. He enjoys hiking, gardening and cooking.

Justin Gildow

Justin Gildow was born and raised in Bellefontaine, Ohio, and is board certified as a Family Nurse Practitioner through the American Nursing Credentialing Center. Justin earned his Bachelor of Science in Nursing at Cedarville University in 2007 and his Master of Science in Nursing from the University of Cincinnati in 2012. Prior to becoming a Nurse Practitioner, Justin worked as a nurse for several years with advanced care and intensive care patients at Miami Valley Hospital in Dayton, Ohio. Justin enjoys caring for the whole person, including their physical, mental, emotional and spiritual aspects. Kristen Davis

Kristen Davis, CNP, is a board-certified Family Nurse Practitioner through the American Academy of Nurse Practitioners. She received her Bachelor of Science in Nursing in 2013 and a Master of Science in Nursing in 2017 from Ohio University. Prior to becoming a nurse practitioner, she was a registered nurse specializing in acute and critical care for almost 10 years. During Kristen's time as a nurse practitioner, she has worked in Internal Medicine and Urgent Care. Kristen loves to interact with all people from every background and cares about the holistic person. She currently lives in Champaign County with her husband and two daughters. Kristen is the youth and children's director at her church. She enjoys fishing, coffee and playing music with her family.

Kristen began her career in 2009 as a Treatment Foster Care therapist at Hogares Inc in Albuquerque, NM. Over the last 10 years, Kristen has continued to work as a therapist as well as a clinical supervisor at an outpatient clinic called Presbyterian Medical Services in Rio Rancho, NM specializing in working with clients of all ages with complex trauma symptoms as well as has extensive experience providing therapeutic supportive services for the LGBTQIA population. Kristen lives in Bellefontaine, Ohio with her wife Sarah, daughter Isabella and two dogs Buckeye and Bubba. In her free time, Kristen loves spending time with her family, kayaking, hiking, and playing competitive board/card games.

Lara Shoemaker

She received her Bachelor of Arts in Social Work from Eastern Mennonite University in Harrisonburg, Virginia, and her Master of Clinical Social Work from The University of Denver, in Denver, Colorado. Lara began her professional career at Adriel School, Inc, working with youth in foster care and group home settings who had severe emotional and behavioral issues. She was promoted to Director of Clinical Services at Adriel in 2015, serving there until joining Community Health & Wellness Partners. Lara is passionate about working with kids, teens, and adults in collaboration with their primary care provider to address mental health needs, such as anxiety, depression, stress, and ADHD. Originally from Archbold, Ohio, Lara lives in West Liberty with her husband, Josh, their son Luke, and dog Piper. Lara enjoys spending time with her family, reading and trying new recipes.

Knight School of Nursing at Ohio Hi-Point Career Center, and in 2011 she earned her Bachelor of Science in Nursing from The Ohio State University. During her pursuit of becoming a Family Nurse Practitioner at Wright State University, she completed two clinical training rotations at Community Health & Wellness Partners. Paula is passionate about serving the community with a focus on the healthcare needs of the whole person. She enjoys working with people of all ages to maintain health and prevent disease, as well as caring for acute and chronic illnesses. Paula lives in Logan County with her husband and enjoys reading, gardening, traveling and a new hobby pursuit - pottery.

Sabrina Salupo

Sabrina Salupo is a certified Physician Assistant through the National Commission on Certification of Physician Assistants. Sabrina is a recent graduate and this will be her first job as a psychiatric certified physician assistant. She is excited to be joining the team at Community Health & Wellness to be putting her education and training to use in helping patients within the community get the healthcare they deserve. She has an interest in mental health and addiction medicine and is passionate to be working within rural healthcare. She wants to help her patients obtain self-care and wellness by providing them with pharmacological and non-pharmacological therapies. She loves spending time with her rescue dog Opie, crafting, flipping furniture and making home décor. She currently resides in Columbus, Ohio with her fiancé Brian.

Sanchita Dhond

Tiffany Hemmert

Prior to becoming a nurse practitioner, Tiffany spent nine years as a Medical Laboratory Scientist working at multiple hospital laboratories across the state of Ohio. Although originally from Cleveland, Ohio, Tiffany currently resides in rural Logan County with her husband and two children. Tracy Detwiler is a certified Physician Assistant through the National Commission on Certification of Physician Assistants. She is a graduate of Benjamin Logan High School. She earned a Bachelor of Science in Health Care Professions with an emphasis on Physician Assistant Studies at the Kettering College of Medical Arts. While a student there she received more than 2,000 clinical hours in both general medicine and specialty practices. As a student, she had several clinical rotations in Logan County. She then earned her Master of Physician Assistant Studies at the University of Nebraska. She spent the first six years of her practice in Clark County in the area of primary care serving patients of all ages. While in primary care, she developed a passion and interest in behavioral medicine. In 2012, she transitioned out of primary care and into psychiatry with an interest in both mental health and addiction medicine in the adult population and served both Logan and Champaign counties. Tracy is a member of the American Academy of Physician Assistants. She has an interest in promoting self-care and wellness, using both pharmacological and nonpharmacological means to recovery. She and her husband live in rural Champaign County with their two daughters. Tracy enjoys spending time with her family, watching OSU football and attending church.

Tracy McPherson, is a Licensed Professional Clinical Counselor who is board certified through the State of Ohio Counselor, Social Worker, Marriage & Family Counseling Board. After an established career in the world of business, she felt called to walk with others through their mental health and addictions needs. To do so, she earned her Master's in Clinical Mental Health Counseling from Ashland Theological Seminary in 2018. She has a strong desire to promote client self-care and wellness, working collaboratively with clients, their medical providers and fellow clinical staff toward recovery in such areas as anxiety, depression, posttraumatic stress and relapse prevention. She and her husband and daughter live in rural Logan County. Tracy enjoys spending time with her family, anything outside, baking, and actively participating within her community and church.

Zoe Henderson

She earned a Bachelor of Science in Biology at Kent State University and a Master of Science in Physician Assistant Studies at Slippery Rock University. During her clinical rotations, she solidified her passion for mental health. She is a member of the American Academy of Physician Assistants and the Ohio Association of Physician Assistants. Zoe is committed to serving the community and bringing awareness to mental health. She and her husband live in Logan County. Zoe enjoys spending time outdoors, reading, and chasing around her three dogs.
