Initially a provider of home health services, the HHCP family of services comprises home health and hospice. Team members include physicians, nurses, certified nurses' aides, therapists, social workers and chaplains. Through our breadth of services and numerous collaborative partnerships, we are able to deliver a full continuum of medical and health services directly to our patient's home... Our services are patient driven, and we are diligent in facilitating continuity of care between the patient's physician(s) and/or other involved providers...
