Dr.Mangione:CommunityengagementspansallofthemissionsofboththeDavidGeffenSchoolofMedicineatUCLAandUCLAHealthSystem-teaching,patient-careandresearch.Throughcommunityengagement-partneringwithorganizationsoutsideoftheuniversity-wehaveaccesstovenueswherewecantrainthenextgenerationofhealthcareprofessionals,andwheretheycanmoredirectlylearnaboutourcommunitiesandhowbesttoimprovethehealthofthepeoplelivinginthosecommunities.Weconductcommunity-partneredresearchwiththeseorganizations,andwedeliverprimary-careservicestothebroadspectrumofpeoplelivingthroughoutthisregion,aswellastertiaryandquaternarycarethatengageswithanevenlargerglobalcommunity.Sowhenwespeakofcommunityengagement,whatwearespeakingabouttrulydoesnothavegeographicboundaries;itisallofthosepeopleweprovidepatient-careto;itinvolvespeopleworkingwithinourvariousservice-learningandresearchpartnerships;itinvolvesourdonorcommunity,whosecreativityandgenerosityarewhat'sgoingtohelpusmaintainexcellenceandstaynationallycompetitiveasourstatefaceshugefinancialshortfalls.
Dr.Mangione:Ithinkitrepresentssomethingofamajorseachange.Foronething,oneofourprioritiesistocreateanewcareerpaththatwillrewardfacultywhodocommunity-partneredresearch.Traditionally,thepersonworkinginanacademicmedicalcenterwhogetsthemostrewardisthebenchscientist-theverysmartindividualtacklingasmallproblem,thinkingcreatively,testingthingsoutinthelab,andcomingupwithacureoranewwaytofindortreataseriouscondition.Thatsingle-scientistmodelhistoricallyhasbeenatthetopofthepyramidinacademicmedicalcenters,andwillcontinuetobehighlyvalued.Butthatisnotthewaycommunity-partneredresearchworks.Community-partneredresearchishighlycollaborative.Generally,itdealswithsuchthingsasgettingthecareefficientlytothepeoplewhoneeditmost.Forexample,wehaveallthesegreatscientificdiscoveriesandwe'velearnedsomuchabouthowtotakecareofpeople,butwestillhaveanobesityepidemicinthiscommunityandsomeofthehighestratesofinfantmortalityinthecountrywithin10-to-15milesfromUCLA.Whyisthat?Community-partneredresearchersaredeeplydedicatedtofindingoutwhatisdrivingthehugedisparitiesinaccessandoutcomesforsomuchofourpopulation.Thesescientistsask,why,whenweknowsomuchabouthowbesttotakecareofpeople,mostpatientsarenotbenefitting?Thiskindofscienceengagesaverybroadspectrumofdisciplines.Youcan'tjustsitinanofficeanddeveloptheworld'sbestdiabetesself-managementinterventionandputitinanicelittlebookletandthengoouttotheseniorcentersandsayhere,dothis.Youhavetositintheroomwiththeclientsandwiththecenterdirectors,andyouhavetounderstandbarriers,thefacilitators;youbuildprogramstogether,testthemtogether,andevaluatethemtogether.Thistypeofcarefullyperformed,community-engagedresearchisaprettybigparadigmshiftformostacademicmedicalcenters,butisessentialifwearegoingtoimprovethehealthofpeopleinLosAngeles.
Dr.Mangione:Thatiscorrect.Historically,schoolsofmedicinehavethoughtabouttheworkforceintermsofdoctors.Butweneedtohaveabroaderview.Wearethinkingaboutphysicians,ofcourse,butwe'realsothinkingaboutnursing,pharmacyandthenawholegroupofpeopleintheallied-healthcarefields-thepharmacytechnicians,thephlebotomists,themedicalassistants,thephysicaltherapistsandothers.Ifyoulookatcurrentprojections,ourhospitalsinWestwoodandSantaMonicaaregoingtofaceverysevereshortagesofpeopleintheallied-healthcareprofessions.Theneedrightnowinourstateandnationallywelloutstripsthenumberofpeoplebeingtrainedinthesefields.SothatistheproblemwefacehereontheWestsideofLosAngeles.Andifwetravel10mileseastandsouth,wewouldbeincommunitieswithsomeofthehighestunemploymentratesinthecountry.Weknowthatlowersocioeconomicstatusandunemploymentcreatetremendousbarrierstoreceivinghealthcareservices.Soifwethinkcreatively,wecansolvethesetwoproblemssimultaneously.
Dr.Mangione:Ihopethatwehaveexcitedourstudentsaboutthepossibilitiesofworkinginacommunity-engagedsetting,andthatourstudentshaveplentyofrolemodelsfromwhomtheycanlearn.Ihopewehaveengageddonorsandphilanthropiststopartnerwithusonthecommunity-engagementmission,sotheyseethemselvesasapartofthemultidisciplinaryteamneededtosuccessfullydothiswork.Ihopeintheacademicpromotionprocess,facultywhospendhundredsofhoursworkingonpartneredprojectsareasvaluedasthebenchscientistsandthatthere'saviablepromotiontrackforthem.AndIhopeIwillneveragaingointoacommunityandhearstoriesabouthowsomepreviousUCLAresearcherscamein,liketheydroppedinoffahelicopter,madeabunchofpromisesthattheydidn'tkeep,tookthedataawayandneversharedtheresultswiththepeoplewhogenerouslyparticipatedintheirresearch.Ihopeinallofoureffortsinthefuture,thefirstpeoplewhohearaboutourresultsandwhobenefitfromthemarethosewhoarethealtruisticpartnerswhojoinedusinourstudiestoimprovethehealthofeveryoneinourcommunity.
Dr.Mangione:Yes.Thereareanumberofreasonswhycommunityengagementhasbeenelevatedtothislevel.First,itisclearthatifapartofourmissionatUCLAistoimprovethehealthofthepeopleofLosAngeles,thenwehavetobeabletonotonlydevelopnewmodelsandbest-practicesforhealthcarebasedonlaboratoryandclinicalresearchbutalsotodeliveronthemmorequickly.Todothat,wehavetoengageourcommunities.Wehavetounderstandwhatthebarrierstocareareinsomeinstances,andalsowhatarethefacilitatorsofcareinothers.Therealityofcaredeliverycan'treallybeappreciatedunlessyoutraindoctors,nursesandalliedhealthprofessionalsinsettingswhereyouseethefullspectrumofpeopleandthechallengesthattheyfacewhenmanagingtheirconditions.
