Dr. Camille Hadida

Dr. Hadida is a licensed Doctor of Chiropractic (1994) and Fellow of the College of Chiropractic Sciences, Canada (1997). Dr. Hadida has specialized in the treatment and evaluation of musculoskeletal injuries and chronic illness since 1996. Dr. Hadida's work experience has afforded her the opportunity to peer review thousands of reports prepared by multidisciplinary health and medical specialists over the past fifteen years. Dr. Hadida has extensive experience in the areas of human function and occupational profiles. Dr. Hadida has worked in the field of Functional Assessments in the home, at the workplace and through facility based assessments since 1996, including training, research and development and preparing educational materials for two of the industry's leading developers of Functional Assessment tools, BTE Technologies (formerly Hanoun Medical Inc.) and VerNova Inc. (formerly ARCON Canada). Dr. Hadida is a Certified VerNova/ARCON evaluator. Dr. Hadida has extensive experience in assessing loss of services in the areas of occupation (vocation), housekeeping, caregiving, attendant care and long term needs for persons with musculoskeletal impairments, including chronic and catastrophic impairments. In 2007, Dr. Hadida completed the academic requirements for certification in Life Care Planning through the University of Carlton's Sprott School of Business and Canadian Institute for Life Care Planners. Dr. Hadida holds current WSIB approved certifications in Adult CPR, Standard First-Aid, Child CPR, Child First-Aid, Automated External Defibrillation and Post Traumatic Debriefment Counseling. In 2011, Dr. Hadida completed the General Aptitude Test Battery Certification Course. Dr. Hadida is currently enrolled in a Forensic Vocational Rehabilitation program at The George Washington University.

Dr. Dimitrios Polyvos

Dr. Polyvos is a graduate of the Canadian Memorial Chiropractic College. He has been practicing Chiropractic as a fully active member of the College of Chiropractors of Ontario in good standing since 2005. Prior to entering the Chiropractic Field, Dr. Polyvos completed his Bachelor of Science in Chemistry at the University of Western Ontario and subsequently obtained his Degree of Master of Science in Chemistry at John Carroll University. After completing his 4 years of Chiropractic training at CMCC, Dr. Polyvos also pursued two additional years of continued education in the Clinical Sciences Program. He has been in private practice since 2005 where he practices and directs a multi-disciplinary clinic in Newmarket, Ontario. Dr. Polyvos has been actively engaged in research studies in the Chiropractic field both at CMCC and Hamilton General Hospital. As a component of his residency program, he was engaged in the observation of pre-surgical consultation of Orthopaedic Spine Surgery under the direction of Dr. Brian Drew at Hamilton General Hospital and Post-Surgical follow-up assessments for upper and lower extremity trauma surgery under the direction of Dr. Dr. Polyvos has also worked at Husky Injection Molding Inc. in Bolton where he was actively engaged in the assessment and management of work related injuries. He has also served as a professor at Seneca College and Seneca at York University. In addition to Private Practice, Dr. Polyvos has been providing services as an independent Chiropractic examiner for 7 years. He has extensive experience in performing independent chiropractic examinations. He has experience in in-home assessments, medico-legal reports, paper-reviews, in-person chiropractic examinations, and functional abilities evaluations. He has been certified as an independent chiropractic evaluator from the Canadian Society of Chiropractic Evaluators and is Internationally Certified in Impairment and Disability Rating based on the AMA Guides to the Evaluation of Permanent Impairment 4th Edition by the American Board of Forensic Professionals.

Dr. Howard Vernon

Dr. Vernon graduated from CMCC in 1977, completed his Fellowship in Clinical Science in 1980 and received his Ph.D. from the University of Glamorgan, Wales, UK, in 2003. He is currently a Full Professor at the Canadian Memorial Chiropractic College where he has worked since 1977. He has published over 100 articles and chapters and has edited two books on the upper cervical spine. He has conducted independent assessments since 1994 and was the leader of the Canadian Memorial Chiropractic College rehabilitation clinic (1998-1994) and the CMCC Disability Assessment Clinic (1996-1998).

Dr. Igor Seiman

Dr. Igor Steiman graduated from the Canadian Memorial Chiropractic College (CMCC) in 1981, having earned a Masters degree in physiology at the University of Toronto in 1978. He then completed a two-year residency in chiropractic clinical sciences at CMCC, and became a Fellow of the College of Chiropractic Sciences (Can.) (FCCS[C]) in 1986. More recently, he has completed certification in clinical education through the University of Toronto's Centre for Faculty Development, and in Interprofessional Education and Collaborative Care.

Dr. Igor Steiman

Dr. Steiman has been in private practice in multi-disciplinary settings and a CMCC faculty member since 1981. At CMCC, he has instructed in various Divisions, conducted and supervised research, held administrative posts, and served on numerous committees. He has authored several scientific and professional papers, and is a chiropractic consultant for various organizations involved in independent assessments. Since 2004, Dr. Steiman has been seconded to the chiropractic clinic of St. Michael's Hospital, Toronto. Besides providing patient care there, he is involved in instructing and mentoring students and residents of various healthcare disciplines. He serves as a chiropractic professional liaison and education leader, and is part of the award-winning Interprofessional Education Working Group. He was awarded the Ontario Chiropractic Association Professional Service Award in 2008.

Dr. Jacob Scheer

Dr. Scheer is a licensed Doctor of Chiropractic (Canadian Memorial Chiropractic College, 1979); a licensed Naturopathic Doctor (Canadian College of Naturopathic Medicine, 1987); and holds a Masters in Health Science - Bioethics (University of Toronto, Faculty of Medicine, 2010). He was a teacher and clinic supervisor at the Canadian College of Naturopathic Medicine from 1987 - 2004. Since 2000 he has been a consultant to various rehabilitation and assessment companies, and DAC centre's in the areas of peer and paper review and Independent Medical Evaluations for motour vehicle accident cases. Dr. Scheer has attended Research Ethics Board meetings at: Toronto Rehab; Centre for Addiction and Mental Health; and Humber River Regional Hospital - Finch and Church site. He is a peer reviewer for Integrated Healthcare Practitioners journal. He has been in private practice seen 1979. He has lectured and written papers on: therapeutic treatment of pain; treatment of musculo-skeletal injuries; management of sport injuries; and H1N1.

Dr. Jerry Grod

Dr. Grod has been a member of the National Board of Chiropractic Examiners (NBCE) test committee for 13 years. Presently, he is a Professional Chiropractic Consultant for the NBCE. He was Deputy Registrar and Registrar for the regulatory body in Ontario (1995-1998). As well, he served as a Director on the Board of the Ontario Chiropractic Association (1994-95) and was the first chiropractor to be a Director on the Board of the Osteoporosis Society of Canada. Dr. Grod is a Certified Association Executive with the Canadian Society of Association Executives. He held the rank of Professor and Director of Continuing Education at CMCC and is currently an adjunct professor at both the New York College of Chiropractic and the Southern California University of Health Sciences.

Dr. John A. Papa

Dr. John A. Papa is a licensed Chiropractor in good standing with the College of Chiropractors of Ontario (CCO). He graduated from the Canadian Memorial Chiropractic College and is also a graduate of the McMaster Contemporary Medical Acupuncture Program. Dr. Papa is internationally certified in the American Medical Association's Guide to Impairment Rating, 4th edition. He has completed three years of post-graduate specialty training in the Chiropractic Rehabilitation Fellowship Program. Dr. Papa is the clinic director and founder of the New Hamburg Wellness Centre where he practices full-time. He is actively involved in the management and treatment of patients with a variety of musculoskeletal complaints including work, sport, and motor vehicle accident injuries. He is also an approved provider of the Workplace Safety and Insurance Board (WSIB) Programs of Care for Acute Low Back Injuries, Upper and Lower Extremity Injuries, and Mild Traumatic Brain Injuries. In addition to treating patients in private practice, Dr. Papa serves as an Independent Chiropractic Consultant for several Disability Management and Assessment Companies. The consulting services provided by Dr. Papa include: Independent Chiropractic Examinations (Treatment and Assessment Plan Physical Assessments to determine entitlement to specified benefits); MIG Determination Physical Examinations; Post 104 Chiropractic Examinations; Catastrophic Determination Screening File Reviews; Non-Catastrophic Paper File Reviews; Functional Assessment Screens; and Executive Summaries. Dr. Papa holds professional membership status with the following organizations: Ontario Chiropractic Association (OCA), Canadian Chiropractic Association (CCA), Canadian Memorial Chiropractic College (CMCC), Canadian Contemporary Acupuncture Association (CCAA), and the Canadian Society of Chiropractic Evaluators (CSCE). Dr. Paul Nolet

Dr. Nolet has practiced as a chiropractor in both Ontario and Harare, Zimbabwe. He is a graduate of the Canadian Memorial Chiropractic College where he also presently an Adjunct Professor in the Department of Graduate Education and Research. Dr. Nolet is a Fellow of the Royal College of Chiropractic Sports Sciences (Canada) and has the International Chiropractic Sports Science Diploma. He has a Master of Science degree in Athletic Training from Life University and a Master of Public Health degree in Health Studies from Lakehead University. He completed a Master's thesis on neck injury in a motor vehicle collision at the Centre of Research Expertise in Improved Disability Outcomes (CREIDO) at Toronto Western Hospital. Dr. Nolet's research interests are in area of neck pain. He has published population-based cohort studies on work-related neck injury and neck injury in a motor vehicle collision and the association with future neck pain. His research has been published in journals such as the European Spine Journal, Journal of Manipulative and Physiological Therapeutics, Pain and the Journal of the Canadian Chiropractic Association. Dr. Nolet practices full-time as a chiropractor at Wellington Orthopaedic and Rehabilitation in Guelph, Ontario. The Clinic is a multidisciplinary clinic with medical specialists, chiropractic, physiotherapy, massage therapy and athletic therapy.

Dr. Philip Decina

Dr. Philip Decina is an Assistant Professor and Clinical Supervisor at the Sherbourne Health Center Chiropractic Clinic which is part of the Canadian Memorial Chiropractic College. He was previously Assistant Clinical Professor for the Herbert K. Lee Clinic at the Canadian Memorial Chiropractic College and was later a Med/Rehab and Disability DAC Assessor with the Rehabilitation Clinic's (TRC) Assessment Center, a Division of CMCC. In 1989, Dr. Decina completed his Doctor of Chiropractic from the Canadian Memorial Chiropractic College and then completed a Fellowship with the College of Chiropractic Sciences (Canada). Dr. Decina's research has focused on the effect of HIV/AIDS and poverty on ill patients. In 2003, he was funded for research on complementary and alternative medicine for the treatment of HIV by the Ontario HIV Treatment Network. He has contributed articles to the Journal of Manipulative and Physiological Therapeutics, Canadian Chiropractor and other academic journals. He has been a member of the examining board for the College of Chiropractic Sciences (Canada) and a member of the Interviewing Team for the CMCC Admissions Department. He is recipient of the Platform Presentation Award by the Conference on Research and Education and has been awarded by the Canadian Foundation for Spinal Research for the First Award for the Student Research Program. Dr. Decina is the CMCC representative on the National Guideline Development Committee and has contributed to the development of the Evidence-based Guidelines for the Chiropractic management of Headache disorders, including Chronic and Episodic Migraine, Cervicogenic and Tension-Type Headaches, published in JMPT in June, 2011 and has also contributed to the development of the Practitioners Guide for the same Headache Guidelines. He will continue to work on this committee as they develop future Practice Guidelines for the Chiropractic Profession.

Dr. Robert Fera

Dr. Fera has been a licensed Chiropractor since 1991. He obtained his B.Sc. in Human Kinetics from the University of Guelph in 1987, his Doctor of Chiropractic designation from CMCC in 1991, and has been certified in Acupuncture since 1996 by the Acupuncture Council of Ontario. Originally from Sudbury, Ontario, Dr. Fera has worked as a Race Medical Assistant on the INDY, IMSA and CART race car circuits. He has participated in a number of crash reconstructions nvolving the use of live test subjects and the BioRid II dummy. He currently is the cast chiropractor for the Sudbury Theatre Centre and Theatre Cambrian, and is the Clinical Director of Southside Health and Rehabilitation. Dr. Fera additionally is a chiropractic consultant working on a multidisciplinary assessment team at the Sudbury Regional Hospital. He is also the Post Offer of Employment assessment consultant for Weston Bakeries and Canada Post, Sudbury Divisions. He conducts Functional Abilities Evaluations, with training in the BTE protocol and he maintains an active clinical practice with extensive experience in motor vehicle collision related injury diagnosis and management. Dr. Fera has lectured to the University of Ottawa Family Medicine Residency Program and has presented at various Geriatrics Conferences, including topics on Trends for Pain Management in Long Term Care. Dr. Fera has been participant in the selection committee for candidates applying for the Midwifery Program at Laurentian University, and has served on various Boards including the Sudbury Brain Injury Association, Sudbury Theatre Center, Science North Foundation, and Sudbury Regional Hospital Fundraising - Professional Cabinet. He has also acted as a preceptor for the Nurse Practitioner Program at the Laurentian University School of Health Sciences and has hosted Family Physician Resident Rounds for the Northern Ontario School of Medicine.

Ms. Amy Babcock

Ms. Amy Babcock is a duly qualified, Masters level Social Worker in the Province of Ontario with more than 10 years of post-degree experience. She is a member in good standing of the Ontario College of Social Workers and Social Service Workers (OCCSWSSW), and the Ontario Association of Social Workers (OASW). She is currently employed in a full-time capacity as a Registered Social Worker in a hospital-based family practice clinic. Her role involves direct clinical work with patients, as well as teaching and providing consultation to medical staff regarding psychosocial matters, treatment options, and community resources. Since 2008, She has been functioning in the role of third party Assessor in the auto insurance industry, providing informed/professional opinions on proposals for Social Work assessments and treatment. She is familiar with current SABS guidelines and ‘best practice' standards within her disciplinary scope. She participates in advanced training opportunities in the MVA sector, and obtain regular supervision from a senior auto insurance Educator/Social Worker.
