Brittany Huckaby

After receiving her Bachelor of Science in Nursing from the University of Central Arkansas, Brittany worked as a pediatric cardiac intensive care nurse for over eight years at Arkansas Children's Hospital, Children's Medical City Dallas, and Children's Health Dallas. While caring for critically ill children and their families, Brittany recognized the increased needs in the mental health field. It was at that point she decided to pursue her Master of Science in Nursing degree and graduated from the University of Texas at Arlington. She is now a Board Certified Psychiatric Mental Health Nurse Practitioner. Brittany's goal is to provide holistic and compassionate care that allow patients to be fully well. In her spare time, she enjoys spending time with friends and family, being outdoors, and cheering on the Arkansas Razorbacks.

Dr. Louis E. Costello

Louis E. Costello, M.D. is board certified in Child, Adolescent, and Adult Psychiatry and has been in practice in the DFW, North Texas area since 1996. He has been practicing in his current location in Highland Village, Texas since 2012. Dr. Costello trained at University of Texas Southwestern in Psychiatry, and he is a Fellow in the American Psychiatric Association. He is recognized for his continued dedication to excellence in clinical care and makes every effort to use the most up-to-date psychiatric assessment, medication management and treatment strategies which are conservative, compassionate, and research-based. Dr. Costello's mission to provide the highest quality medical care to his patients, he endeavors to create a professional and calming environment at The Costello Clinic resulting in the most comprehensive and individualized psychiatric care which includes the following: evaluations, referrals to other medical and psychological caregivers, individual therapy and parent and family guidance. Dr. Costello's goal is for each patient to become as reasonably joyful and functional as their individual strengths will allow them to be. Dr. Louis E. Costello is a Board Certified Psychiatrist with a specialization in Child Psychiatry. At The Costello Clinic we also treat adolescents and adults on a daily basis. The Costello Clinic operates using a Nurse Practitioner model. Dr. Costello meets with the patient for an initial one hour appointment; and, during his evaluation he determines the patient's diagnosis. After the first appointment, the patient will then be paired with one of our Psychiatric Mental Health Nurse Practitioners, whom the patient will see on an ongoing basis. Prior to meeting with the patient for their initial appointment, Dr. Costello has each patient complete an online diagnostic questionnaire. The questionnaire is a smart test which takes approximately two hours to complete. As one question is answered, the next question is determined based on the answer given to the previous question. This allows the questionnaire to focus on specific issues a patient may be dealing with. Many of us have multiple concerns and this questionnaire allows each issue to be fully explored. The questionnaire generates a 22-page report which Dr. Costello reviews prior to the first appointment with each new patient. This questionnaire may be completed at home or here in our office, for your convenience. If the patient is 18 years or older, they will complete the questionnaire themselves. If the patient is younger than 18, the parent or legal guardian will be responsible for completing the questionnaire on the patient's behalf. When you call our office to schedule your first appointment with us, we will inquire as to you your preference regarding taking the questionnaire at home or here in our office.

Meredith Hake

Meredith Hake, RN, FPMHNP completed her Master of Science in Nursing in 2007, and is a Board Certified Family Psychiatric Mental Health Nurse Practitioner. She received an Honors Bachelor of Science in Nursing degree from the University of Texas at Arlington, and a Bachelor of Arts in Psychology and Spanish from the University of Texas at Austin. She has spent the last 10 years working extensively with patients of all ages with depression, anxiety, autism, PTSD, bipolar disorder, ADHD, and other mental health disorders. Prior to completing her Master's degree she worked for 8 years with children and their families at Cook Children's Hospital in Fort Worth, TX. Meredith has worked with Dr. Costello at The Costello Clinic since 2010. Outside of work, Meredith enjoys spending time with her husband and two children. She is actively involved with her children's school as well as her church and her community. She is also involved with the Nursing Program at the University of Texas at Arlington, working to help train future Psychiatric Nurse Practitioners.

Patricia Brooks

Patricia is a Board Certified Psychiatric Mental Health Nurse Practitioner caring for patients of all ages. She is the newest member of The Costello Clinic, joining the team in 2018. She graduated with a Master of Science in Nursing from University of Texas at Arlington and received her Bachelor of Science in Nursing from University of California Irvine, graduating Magna Cum Laude. Prior to entering the field of psychiatry, she spent most of her nursing career working in the Neurological Intensive Care Unit, caring for patients with strokes or traumatic brain injuries, and supporting their families. Patricia enjoys spending time with her family when she is not working. She has also served as Secretary for the Psychiatric Mental Health Nurse Practitioner Association of North Texas and as an area representative for the Psychiatric Mental Health Nurse Practitioner Association of Texas.
