Next Steps has been created by the Dementia Change Action Network (DCAN), a collaboration between Alzheimer's Society, NHS England & Improvement, and the Coalition for Personalised Care... Next Steps helps you to find the right support, at the right time, while waiting for your memory assessment appointment.
