Avelina Cumbis founded Pathfinder Health on May 25th, 1993. It was incorporated on October 2010 under the name of ALC Health Care Inc. On January 2016, ARTICLE 1, THE HOME CARE SERVICES CONSUMER PROTECTION ACT, California Health and Safety Code section 1796 et seq. requires the California Department of Social Services to license Home Care Organization and Registered Home Care Aides. On May 2016 License was issued to Pathfinder health... We provide the finest possible care for our clients to remain independent in their own home. We achieved this by having a team of home care aides (caregivers) who are fully trained at our facility to provide services that are vital to the health and well-being of our clients... Our care plan describes the client's health and physical limitations. We establish a long and short-term goal that each member of the Pathfinder Health Care team must use to either reach their goal or assist in recuperation, surgery or illness.
