In his preceptor role for Medtronic, Dr. Forman is a member of a select group of physicians entrusted with training other doctors and clinicians in the Venous ClosureFast™(RFA) procedure and Medtronic's new VenaSeal Closure™ system. Along with his role as the director of Performance Vein Institute of Los Alamitos, Dr. Forman is managing partner of Los Alamitos Cardiovascular. In December 2015, he completed a two-year term as chief of staff at Los Alamitos Medical Center (LAMC), and since 2014, Dr. Forman has been the director of LAMC's Cardiac Catheterization Lab. In 2018, Dr. Forman became chairperson of LAMC's Board of Governors. Dr. Forman has had some of his research published in American Journal of Cardiology and Catheter Cardiovascular Intervention. He is also a frequent community event guest speaker and contributor to regional publications.
