Sustainable Health Systems is a physician-led team that has designed and developed software to meet the unique operational needs of a clinical service line geared towards lifestyle interventions for disease reversal and health promotion. Through 100+ years of aggregate experience, our leadership has top line and bottom line expertise in program design and operations, providing patient care, assessing population health and in addressing social and behavioral determinants of health. Our perspective, through years of doing NIH-funded basic science and clinical research, is that our systems must incorporate research-grade tools at the lowest possible cost, hence our use of mostly public-domain questionnaires from the U.S. Centers for Disease Control. Software and installation services are available in combination with consulting services, or separately, to help the client optimize program design for the client's environment...
