Angela Poole

Angela Poole, Manager of Analytics. Angela joined the DVACO Team in 2021 with more than a decade in healthcare analytics in Southeastern Pennsylvania. Angela has led several teams responsible for supporting business operations, utilization management, and regulatory reporting across multiple lines of business. At the DVACO, Angela continues to lead analytics by ensuring the team's work aligns with overarching business goals with an eye on innovative solutions. In a life before healthcare, she worked in local government and non-profits. Angela is a graduate of the University of Delaware and holds a Master's in Public Health from Drexel University. Angela lives in Fort Washington, Pennsylvania with her husband, twins, daughter and bernedoodle Dunkin. Most often you'll find her shuttling kids to soccer practice, competing in triathlons, and cheering on her beloved Green Bay Packers.

Beth Souder

Job Titles:

As the Vice President of Clinical Operations, Beth is responsible for the oversight and execution of the care coordination, quality improvement, and post-acute strategies and serves as the clinical lead to payors, health-system leadership, and the community. She provides integral clinical insight and direction to the practice transformation, operations, and analytics teams. Beth brings to the DVACO team extensive clinical, analytical, operational, and Lean Six Sigma Process Improvement experience. Prior to joining the DVACO, Beth served for 17 years in progressive executive operational executive leadership roles at Main Line Health. She holds a BA in Cognitive Science, Artificial Intelligence and an MS in Physical Therapy with active licensure in the State of PA. Beth is an active member of the American Physical Therapy Association and the Inflammatory Bowel Disease Family Research Council at the Children's Hospital of Philadelphia's Center for Pediatric IBD. Beth resides in Skippack PA with her husband, 3 children, and 2 sister rescue cats.

Dr. Mark Angelo - CEO, President

Mark Angelo serves as CEO and President for the Delaware Valley ACO. In this role, he oversees clinical strategy and operations, including quality, population health pharmacy, clinical integration, care coordination, post-acute networks and practice transformation. Dr. Angelo joined the DVACO from Cooper University Health Care where he served many roles including the Medical Director for Population Health and the CEO of the AllCare Health Alliance ACO. In this role, Dr. Angelo maintained oversight of General Internal Medicine and Family Medicine while leading activities for the network of primary care providers surrounding strategy, growth, operations, and innovative models of care. Dr. Angelo was instrumental in the matriculation and participation in alternative payment models at Cooper such as the Oncology Care Model (OCM), the Comprehensive Primary Care Plus (CPC+) Model, and the Bundled Payments for Care Improvement initiative. Dr. Angelo was one of the key stakeholders in launching the AllCare Health Alliance ACO and has led the organization in significant advances in quality performance, cost of care, and particularly in the area of post-acute care management. At Cooper, Dr. Angelo was also the Founding Head of Palliative Medicine within the MD Anderson Cancer Center at Cooper and the Founder and Fellowship Director for the first ACGME accredited fellowship in Hospice and Palliative Medicine in New Jersey. Angelo has been a champion for patient experience at Cooper where great strides have been made in physician communication in the ambulatory setting. Dr. Angelo has delivered talks locally, regionally, and nationally on various topics of palliative medicine, ethics in healthcare, population health, and operational excellence. Angelo is a graduate of Temple University School of Medicine where he also attended internship and residency. He is board certified in palliative medicine and internal medicine and holds a Master's Degree in Healthcare Administration. He is an avid salt-water aquarist, a master of the dad joke, a fitness buff who holds a black belt in Aikido, and is desperately trying to learn how to golf.

Jeanette Flood

Jeanette Flood joined the Delaware Valley ACO in 2015 after years of working with both health plans and health care providers. This experience has given her a unique perspective on how to build relationships on foundations of trust and cooperation. Prior to DVACO, Ms. Flood served in a payer contracting capacity for Jefferson Health System; Thomas Jefferson University Hospital; Cooper University Hospital; and Middlesex Hospital. She spent many years in a variety of capacities with Aetna Inc. where her roles ranged from Systems Analyst to Account Manager for Fortune 100 employers to Executive Director, Aetna Health Plans of California. She also served as Manager, Provider Relations as well as a P&L Plan Manager for provider risk relationships for a large regional HMO. Ms. Flood volunteers in the Delaware Valley for organizations near and dear to her heart including REDI, Inc. and the SPCA. She resides in Montgomery County with her family and one particularly cute dog.

Joel A. Port

Joel A. Port is the Senior Vice President of Business & Network Development of the Delaware Valley ACO, an Accountable Care Organization formed by multiple health systems in southeastern Pennsylvania. In this role, he is responsible for business planning, and network development. Prior to his current position, he held various senior level planning, marketing and network development roles at Main Line Health. Mr. Port is a Fellow of the American College of Healthcare Executives, and assists the local ACHE chapter in setting up an annual educational program in memory of one of his mentors. In addition, he volunteers on multiple committees for The Commission on Accreditation of Healthcare Management Education (CAHME), and is one of their site surveyors, too. He attained his Masters Degree in Health Services Administration from The University of Michigan, earned his B.A. degree from Clark University; and received his CPA in 1987. Joel and his wife, Tammy, have recently relocated to Philadelphia, and are enjoying the "city" life!

Karen Canning

Karen Canning is Manager of Care Coordination at DVACO, providing oversight of the day to day operations of the Care Coordination team. She received her MSN from Wilmington University in 2003 and her Case Management certification in 2014. Karen was born and raised in Delaware County, PA. She feels like she has "done it all" throughout her varied nursing career, including critical care, clinical research trials, management, nursing education, and finally, care coordination. She lives in Glen Mills with her husband, 2 teenagers, her Goldendoodle and 2 cats. Her favorite pastimes are reading, traveling and relaxing with her family.

Lauren Talley

Job Titles:
  • Manager of Practice Transformation & Network Development
In her role as the Manager of Practice Transformation & Network Development, Lauren is responsible for overseeing the daily operations of the Practice Transformation Coaches, who support DVACO's participating practices achieve success transitioning to a value-based care delivery model. Lauren's experience working in an ACO practice, using various EMR systems, clinical workflows and practice operations has proven beneficial in her work supporting member practices. This work includes optimizing patient outcomes by analyzing data and improving processes and operations within the practices. Lauren often collaborates closely with other teams and departments within the DVACO to continue to accelerate the successful transformation to high quality value-based care for our patient populations. Prior to joining the ACO in 2018, Lauren's background included working as a practice manager in various specialties including OB-GYN, Bariatrics, General and Colorectal Surgery. She received her Bachelor of Science from Villanova University in 2013.

Liz Todd

Job Titles:
  • Manager of Care Continuum
As Manager of Care Continuum, Liz Todd is responsible for the development and implementation of programs supporting population health management and quality improvement across the care continuum. She collaborates closely with other clinical departments and health-system leadership to manage relationships with community-based providers. Initially joining the DVACO in May 2020 as the Post-Acute Care Coordinator, she was the DVACO's post-acute clinical resource for physician practices, health system leadership and post-acute providers. Liz has a passion for end-of-life care and the use of clinical data analytics to drive improved patient outcomes. Over her 15+ years of healthcare experience, she has led clinical quality improvement initiatives, clinical education, and interdisciplinary teams. Liz has worked in a variety of settings including acute care, inpatient rehabilitation, outpatient therapy and home health. Originally from Illinois, she received her Doctorate in Physical Therapy from Midwestern University and her Bachelor of Science from Bradley University. Liz lives in Chester County and enjoys reading, traveling, and spending time outdoors with her family.

Monique Bonney

Monique Bonney is an experienced operations manager with over 12 years of experience in healthcare and professional service organizations. Monique's expertise includes project management methodologies, workflow creation, process engineering, and team building. Monique is currently the Director of Population Health Information Technology at the DVACO, one of the largest accountable care organizations in the nation. The DVACO is and actively manages approximately 240,000 Commercial and Medicare patient lives in partnership with Main Line Health System, Jefferson Health System, and Humana. Her role ensures the alignment of the overall IT strategy with clinical and analytical initiatives and compliance requirements. Monique Bonney joined the Delaware Valley ACO in 2017 from Trinity Health where she worked within the project management office. Monique attained her Executive Masters of Business Administration from Drexel University in Philadelphia, PA.

Robert Kagarise

Robert Kagarise's professional experience includes over 25 years of leadership roles in Healthcare, Software and Professional Services organizations. As VP of Operations at the Delaware Valley ACO (DVACO), Rob plans, organizes, directs, and controls all activities of DVACO business (nonclinical) operations and its vendors. He oversees DVACO's information technology, software development, accounting, compliance, security, project management, and facilities functions. Ensures the effective, efficient, reliable and secure deployment and operation of the DVACO's informatics tools which include a wide array of data sources and applications in a primarily Software/Platform as a Service (SaaS) environment. Rob also held leadership roles at Trinity Health within the Population Health, IT Strategic Planning and PMO areas during a ten year span. Under his leadership, Trinity Health was able to successfully develop and deploy enterprise wide population health, interoperability and analytics solutions to meet the challenges of Healthcare Reform. A key focus area for his final 4 years was supporting a number of commercial and Medicare ACOs through the deployment of core capabilities which included a patient\provider portals, quality measures, clinical alerts, point of car registries and a care coordination platform.
