CARE FOR AMERICAS WORKERS

As a CFAW participant, you have 24-hour, 7-day a week access to a member of the Care Coordination Team. Our Team members consist of Registered/Licensed Practical Nurses (Clinical Care Coordinators), Social Workers (Support Coordinators), and Navigators (who assist you in applying, obtaining, and staffing additional assistance you may need in your home). In addition, our Care Coordination Teams work closely with you, your family, caregivers, primary care and ancillary care providers... CFAW professionals work closely with patients and their primary providers (primary providers are typically your primary care physician) to develop a complete health plan.
