Dr. Brandao and Dr. Shahata has relocated to MCI Bloor 345 Bloor St. E, Suite 4A, Toronto ON 416 515 0590. Dr. Turk has relocated to MCI Main Exchange 800 Steeles Ave W, Thornhill ON 905 738 6699. MCI will continue to be the custodian of your medical records. Your medical records are accessible by your doctor at any MCI location. If you decide to receive future medical care with another family physician or walk-in clinic at any of the MCI locations, your physician will have immediate access to your electronic medical records. If you choose to find another family physician outside of the MCI network, you can request a transfer of your medical records accordingly.
